医疗联合体牵头医院医务人员帮扶意愿及工作满意度调查研究 被引量:5

A Study on the Job Satisfaction of Healthcare Staff in Public Hospital under the Reform of Hierarchical Medical System

摘要 目的了解医疗联合体内牵头医院医务人员对开展帮扶工作的意愿及满意度情况,分析存在的问题和障碍。方法在我国东、中、西部9个城市分别抽取1个城市医疗集团和1个县域医共体。将牵头医院调查当日在门诊出诊,工龄在5年以上,且在过去一年内到该医疗联合体内成员单位开展过帮扶工作的医务人员作为调查对象。采用Kruskal Wallis检验和二项logistic回归分析进行统计推断。结果49.9%的医务人员表示非常愿意到下级医疗卫生机构开展帮扶工作。县级医院医务人员工作满意度评价显著高于城市医疗机构。工作负担、收入水平和晋升发展是影响医务人员帮扶意愿的主要因素。结论综合考虑医务人员职业诉求和帮扶需求,提高医务人员下基层帮扶的内生动力;适度减轻医务人员工作负担并提高收入待遇,建立长期可持续的帮扶激励机制;牵头医院为城市二级医院的医疗联合体应更加重视人才队伍和重点专科建设。 Objective To acquire the willingness and satisfaction of medical staff in medical consortium leading hospitals to participate in paired-up assistance,and to define problems and obstacles in this field.Methods An urban medical group and a county medical community were selected in each of the nine cities in the east,central and west of China.Medical staffs who were attending outpatient clinics on the day of the survey at the lead hospital,had at least five years of service and had visited a member unit within that medical association to carry out support work within the past year were taken as survey respondents.Statistical inference was made using Kruskal-Wallis test and twoterm logistics regression.Results 49.9%of the medical staff were willing to carry out paired-up assistance in subordinate institutions.The results of job satisfaction of medical staff in county hospitals were significantly higher than that in urban medical institutions.Workload,income and promotion were the main factors affecting medical staff willingness to participate in paired-up assistance.Conclusion Medical staff career aspirations and support needs should be comprehensively considered,and the internal motivation of medical staff to help at the grassroots level should be increased;the workload of medical staff should be moderately reduced and their income should be improved,and a long-term sustainable incentive mechanism for support should be established;medical associations where the lead hospital is a secondary urban hospital should pay more attention to the construction of talent teams and key specialties.
