摘要
                
                    目的研究宏基因组二代测序(mNGS)在急重症下呼吸道感染性疾病诊断中的应用价值。方法选取西安市中心医院收治的急重症下呼吸道感染性疾病患者90例,收集患者感染部位样本进行mNGS和传统方法检测。记录mNGS和传统方法检测结果。以临床最终诊断为金标准,分别比较mNGS、传统方法对急重症下呼吸道感染性疾病患者不同类型病原体的诊断价值。结果90例患者中,mNGS检测阳性率92%(83/90),混合感染56例(62%)。mNGS检测下,细菌感染检出特异性序列数明显高于支原体、真菌和病毒感染,差异有统计学意义(P<0.05)。90例患者中,传统方法检出阳性率61%,混合感染42例(47%)。mNGS对细菌、病毒、真菌单独诊断的灵敏度均高于传统方法,差异具有统计学意义(P<0.05)。结论与传统方法比较,mNGS能够提高急重症下呼吸道感染性疾病患者病原体的诊断灵敏度,尤其是细菌、真菌和病毒感染。
                
                Objective To explore the application value of metagenomic next-generation sequencing(mNGS)in the diagnosis of acute and severe lower respiratory tract infectious diseases.Methods A total of 90 patients with acute and severe lower respiratory tract infectious diseases admitted to Xi′an Central Hospital were selected,and samples from the infected parts of the patients were collected for mNGS and traditional methods.The results of mNGS and traditional methods were recorded.Taking the final clinical diagnosis as the gold standard,the diagnasitic value of mNGS and traditional methods in diagnosing different types of pathogens in patients with acute and severe lower respiratory tract infectious diseases were compared.Results Among 90 patients,the positive rate of mNGS was 92%(83/90),and 56 patients(62%)had mixed infection.Under mNGS detection,the number of specific sequences detected in bacterial infection was significantly higher than that in mycoplasma,fungal and virus infection,and the difference was statistically significant(P<0.05).Among 90 patients,the positive rate of traditional methods was 61%,and 42 patients(47%)had mixed infections.The sensitivity of mNGS for diagnosing bacteria,viruses and fungi alone were higher than those of traditional methods,and the differences were statistically significant(P<0.05).Conclusion Compared with traditional methods,mNGS can improve the diagnostic sensitivity of pathogens in patients with acute and severe lower respiratory tract infectious diseases,especially bacterial,fungal and viral infections.
