高通量血液透析对糖尿病肾病患者免疫功能、生存质量及预后的影响 被引量:1

Effects of high flux hemodialysis on immune function,quality of life and prognosis of patients with diabetes kidney disease

摘要 目的分析高通量血液透析对糖尿病肾病患者免疫功能、生存质量及预后的影响。方法选择2018年9月至2021年9月我院收治的80例糖尿病肾病患者为研究对象,按红绿双色球法将其分为对照组(n=40,低通量血液透析治疗)和研究组(n=40,高通量血液透析治疗)。比较两组的炎症因子水平、免疫功能指标、氧化应激指标、生存质量评分及预后。结果治疗后,两组的白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、白细胞介素-1β(IL-1β)以及白细胞介素-17(IL-17)水平均较治疗前降低,且研究组低于对照组(P<0.05)。治疗前,两组的白细胞分化抗原3阳性(CD3^(+))、白细胞分化抗原4阳性(CD4^(+))、白细胞分化抗原8阳性(CD8^(+))及CD4^(+)/CD8^(+)比较,差异无统计学意义(P>0.05);治疗后,研究组的CD3^(+)、CD4^(+)及CD4^(+)/CD8^(+)高于对照组和治疗前,CD8^(+)低于对照组和治疗前(P<0.05)。治疗后,研究组的丙二醛(MDA)水平低于对照组和治疗前,超氧化物歧化酶(SOD)、谷胱甘肽过氧化物酶(GSH-Px)、总抗氧化能力(T-AOC)水平高于对照组和治疗前(P<0.05)。治疗后,两组的生存质量评分均较治疗前升高,且研究组高于对照组,差异具有统计学意义(P<0.05)。随访半年,研究组的不良心血管事件发生率低于对照组(P<0.05);两组的死亡率比较,差异无统计学意义(P>0.05)。结论高通量血液透析可显著减轻糖尿病肾病患者的炎症反应、氧化应激反应,提升机体免疫力,减少不良心血管事件的发生,改善患者的生存质量。 Objective To analyze the effects of high flux hemodialysis on immune function,quality of life and prognosis of patients with diabetes kidney disease.Methods Eighty patients with diabetes kidney disease admitted in our hospital from September 2018 to September 2021 were selected as the research objects and divided into control group(n=40,low flux hemodialysis treatment)and study group(n=40,high flux hemodialysis treatment)according to the red-green bicolor method.The levels of inflammatory factors,immune function indicators,oxidative stress indicators,quality of life scores and prognosis were compared between the two groups.Results After treatment,the levels of interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β)and interleukin-17(IL-17)in the two groups were lower than those before treatment,and those in the study group were lower than the control group(P<0.05).Before treatment,there were no significant differences in cluster of differentiation 3 positive(CD3^(+)),cluster of differentiation 4 positive(CD4^(+)),cluster of differentiation 8 positive(CD8^(+))and CD4^(+)/CD8^(+)between the two groups(P>0.05);after treatment,the CD3^(+),CD4^(+)and CD4^(+)/CD8^(+)in the study group were higher than those in the control group and before treatment,while the CD8^(+)was lower than that in the control group and before treatment(P<0.05).After treatment,the level of malondialdehyde(MDA)in the study group was lower than that in the control group and before treatment,and the levels of superoxide dismutase(SOD),glutathione peroxidase(GSH-Px)and total antioxidant capacity(T-AOC)were higher than those in the control group and before treatment(P<0.05).After treatment,the scores of quality of life in both groups were higher than those before treatment,those in the study group were higher than the control group,and the differences were statistically significant(P<0.05).Following up for half a year,the incidence of adverse cardiovascular events in the study group was lower than that in the control group(P<0.05);there was no significant difference in mortality between the two groups(P>0.05).Conclusion High flux hemodialysis can significantly reduce the inflammatory reaction and oxidative stress reaction,promote the immunity of the body,decrease the occurrence of adverse cardiovascular events,and improve the quality of life of patients with diabetes kidney disease.
