去甲肾上腺素联合美托洛尔对重症脓毒性休克患者肾功能及血流动力学的影响 被引量:1

Effects of norepinephrine and metoprolol on renal function and hemodynamics in patients with severe septic shock

摘要 目的观察去甲肾上腺素联合美托洛尔对重症脓毒性休克患者肾功能及血流动力学的影响。方法将50例脓毒性休克患者随机分为单药组(27例)和联合组(23例)。单药组单用去甲肾上腺素治疗,联合组使用去甲肾上腺素联合美托洛尔治疗,比较两组患者的肾功能及血流动力学的变化。结果两组患者治疗24 h后血肌酐(SCr)、血尿素氮(BUN)、心排血指数(CI)、心率(HR)水平显著低于治疗前水平,肌酐清除率(CCr)、每小时尿量、平均动脉压(MAP)、体循环血管阻力指数(SVRI)均较治疗前明显升高(P<0.05),且联合组的血流动力学指标改善程度更显著(P<0.05),而肾功能指标两组比较差异无统计学意义(P>0.05)。结论去甲肾上腺素联合美托洛尔能够明显改善脓毒性休克患者的血流动力学指标,其对肾功能的改善程度与单用去甲肾上腺素相似。 Objective To explore the effect of norepinephrine and metoprolol on renal function and hemodynamics in patients with severe septic shock.Methods Fifty patients with septic shock admitted to our hospital were randomly divided into monotherapy group(27 cases)and combination medicine group(23 cases).The monotherapy group was treated with norepinephrine alone,and the combination medicine group was treated with norepinephrine combined with metoprolol.The renal function and hemodynamic changes of the two groups were compared.Results After 24 hours of treatment,the SCr and BUN levels of the two groups were significantly lower than those before treatment.The CCr level and urine output per hour were higher than before treatment,and the differences were statistically significant(P<0.05).But there was no significant difference between the two groups(P>0.05).The CI and HR levels of the two groups of patients were lower than those before treatment after 24 hours of treatment.The levels of MAP and SVRI were higher than before treatment,and the combination medicine group was better than the monotherapy group(P<0.05).Conclusion The combined use of norepinephrine and metoprolol in the treatment of patients with septic shock can effectively improve the renal function and hemodynamic parameters of the patients.
