摘要
年龄相关性黄斑变性(AMD)是眼科的常见疾病,因玄府不得升降出入表现为视物模糊、变形。而目之神光发越有赖于目络的通畅,目络不通可由虚、实二因导致。基于目络不通,结合患者的症状及眼底表现,阐述目络不通在AMD发病中的作用,从虚实入手,建立特有分期辨证及治疗方法,AMD初期为目络瘀阻阶段,病位在阳络,由气虚无力、血络不荣、清阳不升引起,三者可并存于疾病发展过程中;后期为络息成积阶段,病位在阴络,多由瘀血、痰浊阻滞日久引起,络气结滞,血行不畅,甚或气血运行中断,闭塞不通。治疗时分别选取益气通络、养荣通络、软坚散结通络、化瘀通络、辛温通络等治法,为该病的治疗提供新思路。
Age-related macular degeneration(AMD) is a common disease in ophthalmology,which is characterized by blurred and distorted vision due to dysfunction of XuanFu. The divine light of the eyes depends on free of the eyes collaterals. The blockage of the eyes collaterals can be caused by the two causes as deficiency and excessive syndrome. Based on the obstruction of the eye collateral, combined with the symptoms and fundus manifestations of patients, this paper expounds the role of obstruction of the eye collateral in the pathogenesis of AMD. Starting from the deficiency and excessive, it establishes a unique methodology of staging syndrome differentiation and treatment method. The initial stage of AMD is the phase of obstruction of the eye collateral,and the location of the disease is in the Yang collateral, which is caused by the weakness of Qi, the lack of nurturing of blood collateral, and the clear Yang failing to ascend. The three causses above can coexist in the process of disease development, the late stage is the phase of accumulation caused by collateral obstruction. The disease locates in Yin collaterals,which is mainly caused by blood stasis and phlegm stagnation for a long time. Once the Collateral Qi stagnated and accumulated, it will lead to blood circulation stasis, or even Qi and blood circulation interrupted,and eventually obstruction formed. For treatment, the methods of invigorating Qi and freeing the collateral vessels, nourishing prosperity and unblocking collaterals, softening hardness and dissipating binds to free the collateral vessels, removing blood stasis to the collateral vessels, and pungent-warming to free the collateral vessels are mainly used and these methods provide new ideas for the treatment of the disease.
