摘要
目的探讨间歇导尿联合针刺对脊髓损伤后神经源性膀胱患者膀胱功能的影响。方法选取南昌市洪都中医院2018年6月—2020年5月收治的66例因脊髓损伤而造成神经源性膀胱的患者作为研究对象,按照随机数字表法将其分为参照组(33例)与研究组(33例),参照组采用间歇导尿技术治疗,研究组采用间歇导尿技术联合针刺治疗。比较2组治疗前、治疗后的中医症状积分与膀胱功能恢复情况。结果2组治疗前的中医症状积分比较,差异无统计学意义(P>0.05);2组治疗后的中医症状积分低于治疗前,差异有统计学意义(P<0.05);研究组治疗后的中医症状积分低于参照组,差异有统计学意义(P<0.05)。2组治疗前的膀胱最大排尿量、膀胱顺应性比较,差异无统计学意义(P>0.05);2组治疗后的膀胱最大排尿量、膀胱顺应性高于治疗前,差异有统计学意义(P<0.05);研究组治疗后的膀胱最大排尿量、膀胱顺应性高于参照组,差异有统计学意义(P<0.05)。结论间歇导尿技术联合针灸治疗脊髓损伤后神经源性膀胱,可明显改善患者症状,促进其膀胱功能恢复。
Objective To investigate the effect of intermittent catheterization combined with acupuncture on the recovery of bladder function in patients with neurogenic bladder after spinal cord injury.Methods 66 patients with neurogenic bladder caused by spinal cord injury in Nanchang Hongdu Hospital of Traditional Chinese Medicine from June 2018 to may 2020 were selected as the research objects,and they were randomly divided into control group(33 cases)and the study group(33 cases)according to the digital table methods.The control group was treated with intermittent catheterization,and the study group was treated with intermittent catheterization combined with acupuncture.The traditional Chinese medicine(TCM)syndrome scores and bladder function recovery of the two groups before and after treatment were compared.Results There was no significant difference in TCM syndrome score between the two groups before treatment(P>0.05).The TCM symptom scores of the two groups after treatment were lower than those before treatment,and the difference was statistically significant(P<0.05).After treatment,the TCM syndrome score of the study group was lower than that of the control group,and the difference was statistically significant(P<0.05).There was no significant difference in the maximum bladder volume and bladder compliance between the two groups before treatment(P>0.05).The maximum urine output and bladder compliance of the two groups after treatment were higher than those before treatment,and the differences were statistically significant(P<0.05).After treatment,the maximum urinary volume and bladder compliance of the study group were higher than those of the control group,and the differences were statistically significant(P<0.05).Conclusion Intermittent catheterization combined with acupuncture in the treatment of neurogenic bladder after spinal cord injury can significantly improve the TCM syndrome and promote the recovery of bladder function.
