摘要
本文从黄元御的相关理论及“伏邪”的发病特点出发,得出支气管扩张发病以脾胃阳虚为基础,并认为扶阳宜予四逆加人参汤。支气管扩张常因阳邪盘踞于肺,蛮补阳气,易使肺中邪热滋长,反致病重。笔者结合自身临床经验总结得出,扶阳时视临床病机,辅以通阳泻浊、潜阳固阳以及引火归元,能使扶阳法在支气管扩张治疗中得到安全有效运用。又因支气管扩张发病多由外感诱发,且具有“伏邪”深藏的特点,故在治疗中处理好扶阳与祛外邪、扶阳与祛伏邪的关系亦尤为重要,另在胃土通降有序的前提下,予疏肝通肺络可有效提高临床疗效。笔者认为从扶阳角度探索支气管扩张的治疗,对减少该病反复发作、促进病邪排出以及破损气管的修复具有积极的临床意义。
According to Huang Yuanyu′s thought and the characteristics of latent pathogenic factor,this paper concludes that the pathogenesis of bronchiectasis is based on the deficiency of spleen and stomach Yang,and it is suggested that Sini Renshen Decoction should be used to aid Yang qi.Bronchiectasis is often caused by Yang evil,blind supplement of Yang qi would often make lung evil heat grow and cause disease to worsen.Combined with the authors′clinical experience,it is concluded that Fuyang therapy can be safely and effectively applied in the treatment of bronchiectasis according to the clinical pathogenesis,and supplementing with clearing Yang and removing turbidity,suppressing and strengthening Yang and guiding fire to origin.The pathogenesis of bronchiectasis is mainly induced by external induction and has the characteristics of latent pathogenic factor,so it is particularly important to deal with the relationship between Fuyang and removing external evil,Fuyang and removing latent pathogenic factor during the treatment.In addition,on the premise of the orderly treatment of stomach soil,the treatment of liver and lung can effectively improve the clinical efficacy.The authors believe that exploring the treatment of bronchiectasis from the perspective of Fuyang has positive clinical significance in reducing the recurrence of the disease,promoting the elimination of pathogenic factors and repairing damaged trachea.
