摘要
目的基于Logistic回归分析多发脑转移瘤手术后的预后影响因素。方法回顾性分析2019年1月~2021年5月本院多发脑转移瘤手术后的60例患者的临床资料,记录患者术后的神经系统症状(其中剧烈头痛48例、恶心呕吐42例、癫痫26例、偏瘫18例)改善情况、生存时间及1年、2年生存率,收集患者的临床资料,基于Logistic回归分析多发脑转移瘤手术后的预后危险因素。结果经随访发现,术后41(68.33%)例患者的神经系统症状消失;患者的平均生存时间为(17.02±9.91)个月,1年生存率、2年生存率分别为51.67%(31/60)、36.68%(22/60)。经单因素分析显示,KPS评分、术后放疗、术后靶向治疗、未颅外转移、原发灶控制情况是影响患者预后的可能危险因素(P<0.05)。经多因素Logistic回归分析显示,术前KPS评分<80分、术后放疗、术后靶向治疗、颅外转移、原发灶控制差是影响多发脑转移瘤手术后预后的独立危险因素(P<0.05)。结论KPS评分、术后放疗、术后靶向治疗、颅外转移、原发灶控制情况可影响患者预后,必须根据这些因素在术后采取相应的干预措施。
Objective To analyze the prognostic factors of multiple brain metastases after surgery based on logistic regression.Methods The clinical data of 60 patients after surgery for multiple brain metastases in our hospital from January 2019 to May 2021 were retrospectively analyzed,neurological symptoms,survival time and 1-year and 2-year survival rates after surgery were recorded,and the clinical data of patients were collected.The prognostic risk factors of multiple brain metastases after surgery were analyzed based on logistic regression.Results The neurological symptoms disappeared in 41(68.33%)patients after operation;The mean survival time was(17.02±9.91)months,1-year survival rate was 51.67%(31/60)and 2-year survival rate was 36.68%(22/60),respectively.Univariate analysis showed that KPS score,tumor diameter,postoperative radiotherapy,postoperative targeted therapy,no extracranial metastasis and primary lesion control were the possible risk factors affecting the prognosis of patients(P<0.05).Multivariate logistic regression analysis showed that KPS score<80,postoperative radiotherapy,postoperative targeted therapy,extracranial metastasis and poor primary control were independent risk factors affecting the prognosis of multiple brain metastases after surgery(P<0.05).Conclusion KPS scores,postoperative radiotherapy,postoperative targeted therapy,extracranial metastases,and control of the primary site can affect prognosis of patients with multiple brain metastases.According to these factors,corresponding intervention measures must be taken after operation.
