摘要
目的 探讨经皮腔内血管成形术(PTA)治疗血液透析患者动静脉内瘘狭窄的临床效果。方法 15例血液透析动静脉内瘘狭窄患者,均行PTA治疗。统计狭窄情况、手术成功情况及并发症发生情况,比较手术前后狭窄处内径、峰值流速、肱动脉血流量。结果 15例患者超声检查均发现狭窄,共15处,其中左前臂头静脉9处(左吻合口处合并有血栓),上臂头静脉6处。15例患者手术成功率为100%,术后患者未出现并发症。术后,狭窄处内径、肱动脉血流量大于术前,峰值流速低于术前,差异有统计学意义(P<0.05)。结论 PTA是治疗血液透析患者动静脉内瘘狭窄的有效方法 ,其操作简便、创伤小、可移动性好,易引导穿刺、显像实时性好、近期效果明显,且血流动力学评估好,对血液透析动静脉内瘘的治疗有重要价值。
Objective To discuss the clinical effect of percutaneous transluminal angioplasty(PTA)in the treatment of arteriovenous fistula stenosis in hemodialysis patients.Methods 15 hemodialysis patients with arteriovenous fistula stenosis were treated with PTA,and the stenosis,success rate of surgery and occurrence of complications were counted.The internal diameter of the stenosis,peak flow velocity and brachial artery blood flow before and after surgery were compared.Results Ultrasound in all 15 patients revealed stenoses,15 stenoses in total,including 9 stenoses in the left forearm cephalic vein(combined with thrombus at the left anastomosis)and 6 stenoses in the upper arm cephalic vein.The success rate of the surgery was 100%in 15 patients,and no postoperative complications occurred in patients.After surgery,the internal diameter of the stenosis and brachial artery blood flow were greater than those before surgery,and the peak flow rate was lower than that before surgery,and the differences were statistically significant(P<0.05).Conclusion PTA is an effective method for the treatment of arteriovenous fistula stenosis in hemodialysis patients.It is easy to operate and guide puncture,and has less traumatic,good mobility,good real-time imaging,obvious short-term effect,and good hemodynamic evaluation,which is of great value for the treatment of arteriovenous fistula stenosis in hemodialysis.
