MRI联合MRA在症状性颈动脉狭窄诊断中的应用

Application of Magnetic Resonance Imaging Combined with Magnetic Resonance Angiography in the Diagnosis of Symptomatic Carotid Artery Stenosis

摘要 目的分析磁共振成像(MRI)联合血管造影(MRA)在症状性颈动脉狭窄(CAS)诊断中的应用。方法回顾性分析2021年1月至2022年1月在濮阳市油田总医院收治的60例CAS患者,所有患者均行MRI及MRA检查,且经数字减影血管造影(DSA)确诊。评价MRI联合MRA显示的图像质量;以DSA结果和病理结果作为检验的标准,采用Kappa一致性分析,探讨MRI联合MRA诊断CAS及评估斑块特征的价值。结果所得60例图像中,质量分级为4级者有38例,3级15例,2级5例,1级2例。以DSA的结果作为参照的标准,MRI联合MRA诊断颈动脉轻、中、重度狭窄以及完全闭塞的准确率分别为91.67%、89.58%、95.83%、97.92%,敏感度分别为86.96%、85.71%、93.33%、85.71%,特异度分别为96.00%、91.18%、96.30%、98.88%,Kappa值分别为0.832、0.753、0.850、0.846。对DSA确诊重度狭窄的15支颈动脉行颈动脉内膜剥脱术,MRI联合MRA与术后病理检查显示斑块成分和硬度的情况差异无统计学意义(P>0.05)。结论MRI与MRA联合应用可以准确评估颈动脉的狭窄程度和粥样硬化斑块的特征,应用于CAS的诊断有一定的价值。 Objective To analyze the application of magnetic resonance imaging(MRI)combined with magnetic resonance angiography(MRA)in the diagnosis of symptomatic carotid artery stenosis(CAS).Methods A total of 60 patients with CAS admitted to the hospital from January 2021 to January 2022 were retrospectively analyzed.All patients underwent MRI and MRA,and they were confirmed by digital subtraction angiography(DSA).The images quality by MRI combined with MRA was evaluated.Taking DSA results and pathological results as the test standard,the clinical diagnostic value of MRI combined with MRA for CAS and plaque characteristics was analyzed by Kappa consistency test.Results In the 60 patients,there were 38 cases with images quality at grade 4,15 cases at grade 3,5 cases at grade 2 and 2 cases at grade 1.Taking DSA results as the reference standard,accuracy of MRI combined with MRA in the diagnosis of mild stenosis,moderate stenosis,severe stenosis and complete occlusion were 91.67%,89.58%,95.83%,97.92%,sensitivity were 86.96%,85.71%,93.33%%,85.71%,specificity were 96.00%,91.18%,96.30%,98.88%and Kappa values were 0.832,0.753,0.850,0.846,respectively.Cartid endarterectomy was performed on 15 carotid arteries with severe stenosis confirmed by DSA.There were no significant differences in composition and hardness of plaques by MRI combined with MRA and pathological examination(P>0.05).Conclusion MRI combined with MRA can accurately assess CAS severity and characteristics of atherosclerotic plaques,and has certain diagnostic value for CAS.
