摘要
目的探析心脏冠状动脉血管成像(Cardiac coronary angiography,CCTA)的血管及斑块特征对缺血性狭窄的预测价值。方法选择2021年1月-2022年1月行CCTA检查的110例疑似冠心病患者。根据血流储备分数(FFR)分为缺血组(≤0.80)53例和非缺血组(>0.80)57例。所有患者均行CCTA检查,对比两组患者CCTA血管及斑块特征,采用logistic回归模型分析缺血性狭窄中CCTA特征预测独立因素。绘制受试者工作特征曲线(ROC)评估CCTA血管及斑块特征预测缺血性狭窄的价值。结果缺血组与非缺血组患者在狭窄程度、病变受累血管、斑块长度、弥漫病变支数、重构指数特征对比,差异具有统计学意义(P<0.05);其余血管及斑块特征指标[包括病变部位支数、斑块体积、负荷、钙化斑块体积(CPV)、纤维斑块体积(FPV)及低密度斑块体积]两组对比差异无统计学意义(P>0.05)。logistic回归模型结果显示,狭窄程度(OR=0.689)、弥漫病变(OR=0.325)、病变受累血管特征(OR=0.511)均为缺血性狭窄独立影响因素(P<0.05)。ROC结果显示,狭窄程度、弥漫病变AUC>0.8,且狭窄程度特征诊断效能最高,AUC(95%CI)为0.891(0.798-0.957),灵敏性为87.43%,特异度为91.25%。结论CCTA中狭窄程度、弥漫病变、病变受累血管特征特征为缺血性狭窄独立影响因素,其中狭窄程度、弥漫病变特征诊断预测缺血性狭窄效能较高。
Objective To explore the predictive value of vascular and plaque characteristics of cardiac coronary angiography(CCTA)for ischemic stenosis.Methods A total of 110 patients with suspected coronary heart disease who underwent CCTA examination in our hospital from January 2021 to January 2022 were retrospectively selected.According to the fractional flow reserve(FFR),53 cases were divided into ischemic group(≤0.80)and 57 cases were divided into non-ischemic group(>0.80).All patients underwent CCTA examination.The characteristics of CCTA vessels and plaques were compared between the two groups.Logistic regression model was used to analyze the independent predictors of CCTA characteristics in ischemic stenosis.Receiver operating characteristic(ROC)curve was drawn to evaluate the value of CCTA vascular and plaque features in predicting ischemic stenosis.Results There were significant differences in the stenosis degree,affected vessels,plaque length,number of diffuse lesions,and remodeling index between the ischemic group and the non-ischemic group(P<0.05).There was no significant difference in other vascular and plaque characteristics(including number of lesion sites,plaque volume,load,calcified plaque volume(CPV),fibrous plaque volume(FPV)and low-density plaque volume)between the two groups(P>0.05).The results of logistic regression model showed that the degree of stenosis(OR=0.689),diffuse lesion(OR=0.325),and vascular characteristics involved in the lesion(OR=0.511)were independent influencing factors of ischemic stenosis(P<0.05).ROC results showed that the AUC of stenosis and diffuse lesions was>0.8,and the diagnostic efficiency of stenosis was the highest,with the AUC(95%CI)of 0.891(0.798-0.957),the sensitivity was 87.43%,and the specificity was 91.25%.Conclusions The degree of stenosis,diffuse lesions and the characteristics of vessels involved in CCTA are independent influencing factors of ischemic stenosis,and the degree of stenosis and diffuse lesions have a higher diagnostic efficiency in predicting ischemic stenosis.
