摘要
目的:研究儿童早期预警评分(PEWS)在急诊室危重患儿分流与转运中的应用价值。方法:选取深圳市儿童医院2018年1月-2019年12月儿科急诊收治的480例危重患儿为研究对象,利用随机数表法分为对照组与研究组,每组240例。在分流和转运过程中,对照组实施常规护理管理,研究组基于PEWS评分实施分级护理配合分流与转运。观察并分析两组分流效率(分流时间、候诊时间、分流准确率)、病情控制情况(救治无效死亡或病情恶化、病情稳定)、转运成功率、不良事件发生情况及患儿家属对护理的满意度。结果:研究组分流和候诊所需时间短于对照组(P<0.05),分流准确率高于对照组(P<0.05)。研究组的疾病有效控制率和转运成功率均高于对照组(P<0.05),不良事件发生率低于对照组(P<0.05)。研究组患儿家属满意度高于对照组(P<0.05)。结论:急诊室危重患儿分流与转运中实施PEWS评分管理,可提高分流效率,有助于患儿疾病的有效控制和院际安全转运,并可降低相关不良事件发生率,提升患儿家属的满意度。
Objective:To study the application value of pediatric early warning score (PEWS) in shunt and transfer of critically ill children in emergency room.Method:A total of 480 critically ill children admitted to Pediatric Emergency Department of Shenzhen Children’s Hospital from January 2018 to December 2019 were selected as the research subjects.They were divided into control group and study group by ranom number table method,with 240 cases in each group.In the process of shunt and transfer,the control group received routine nursing management,and the study group received PEWS-based graded nursing to cooperate with shunt and transfer.The shunt efficiency (shunt time,waiting time,shunt accuracy),condition control (ineffective treatment,death or deterioration of condition,stable condition),success rate of transfer,occurrence of adverse events and family members’ satisfaction with nursing were observed and analyzed.Result:The shunt time and waiting time in the study group were shorter than those in the control group (P<0.05),and the shunt accuracy was higher than that in the control group (P<0.05).The effective disease control rate and success rate of transport in the study group were higher than those in the control group (P<0.05),and the incidence of adverse events was lower than that in the control group (P<0.05).Family members’ satisfaction in the study group was higher than that in the control group (P<0.05).Conclusion:The implementation of PEWS management in shunt and transfer of critically ill children in emergency room can improve the shunt efficiency,which is conducive to effective control of the children’s diseases and safe inter-hospital transfer.In addition,it can reduce the incidence of related adverse events and improve the satisfaction of children’s family members.
