摘要
目的探讨改良早期预警评分在儿童院际转运中对危重病情和预后预测的应用价值和可行性。方法以儿童医院120院际转运至ICU的285例急诊患儿为研究对象分别进行早期预警评分(PEWS)和改良早期预警评分(MPEWS),追踪患者的预后。分别比较两种评分不同分数段患儿分布情况;比较两种评分对患儿死亡及患儿入院21 d的病情评价和最终预后的ROC曲线差异。结果存活组和死亡组的PEWS评分低于MPEWS评分,以死亡为预测指标时,PEWS与MPEWS评分的ROC线下面积比较,差异有统计学意义(P<0.05);以入院21 d未愈组为观察指标,PEWS与MPEWS评分的ROC线下面积比较,差异有统计学意义(P<0.05)。结论MPEWS评分系统比PEWS评分系统在院际转诊中对预测儿童预后及转归较为准确,帮助医护人员更方便快捷的识别和处理危重症或潜在危重症患儿,提升患儿安全,有利于合理分配医疗资源,值得临床推广。
Objective To explore the application value and feasibility of improved early warning score in the evaluation of critical illness and prognosis in children’s inter-hospital referral.Methods A total of 285 emergency children who were transferred to the ICU from the Children’s Hospital were enrolled in the study.Early warning score(PEWS)and modified early warning score(MPEWS)were used to track the prognosis of patients.The distribution of children with different scores was compared.The differences of ROC curves between the two scores on the death of the children and the 21-day admission and final prognosis of the children were compared.Results The PEWS scores of the survival group and the death group were lower than the MPEWS score.When the death was used as the predictor,the area under the ROC line of PEWS and MPEWS scores was statistically significant(P<0.05).The 21-day untreated group was used as an observation index.The area under the ROC line of PEWS and MPEWS scores was statistically significant(P<0.05).Conclusion The MPEWS scoring system is more accurate than the PEWS scoring system in predicting children’s prognosis and outcome in inter-hospital referral.It can help medical staff to identify and treat critically ill or potentially critically ill children more easily and quickly,improve the safety of children and Reasonably allocate medical resources.It is worthy of clinical promotion.
