摘要
目的对河南省县级公立医院临床医生抗菌药物应用管理认知和行为及其影响因素进行调查,为科学干预提高临床医生抗菌药物合理使用提供依据。方法问卷调查216名临床医生,定量分析抗菌药物应用行为的影响因素,定性分析县级公立医院抗菌药物应用管理的阻碍因素。结果县级公立医院临床医师获取抗菌药物应用知识的主要来源是学术会议和讲座,有65.3%的医师认为抗菌药物管理制度给自己带来了压力,26.45%的调查对象在面对患者压力时选择会开具抗菌药物,影响这一行为的因素主要有性别和是否愿意遵守该院抗菌药物临床应用管理办法和规定。定性分析发现,临床药师缺乏是县级公立医院抗菌药物应用管理的阻碍因素。结论县级公立医院抗菌药物应用管理需要加强对临床医师的培训,配备临床药师等专业人员,开展以技术为导向的综合管理措施。
Objective To investigate the status and influencing factors of clinicians’ cognition and behavior of antimicrobial use and management in the county public hospitals of Henan Province, and to provide basis for scientific intervention to improve the rational use of antimicrobials by clinicians.Methods A questionnaire survey was conducted among 216 clinicians, and the influencingfactorsforantimicrobialapplication behavior were analyzed by quantitativemethods, and qualitative analysis was used to analyze the hindering factors of application and management of antimicrobials in county public hospitals.Results The study found that the main sources for clinicians to obtain knowledge about antimicrobial usewere academic conferences and lectures. Altogether 65.3% of the physicians believed that the antimicrobial management system brought pressure to them, and 26.45% of the respondents chose to prescribe antibiotics when faced with the pressure frompatients.The main factors affecting this behavior were gender and willingness to comply with the hospital’s antimicrobial management rules and regulations. Qualitative analysis found that lack of clinical pharmacists was a hindering factor forthe application and management of antibiotics in county public hospitals. Conclusions The study indicates that county public hospitals need to strengthen its training of clinicians and the appointment of clinical pharmacists and other professionals to carry out technique-oriented comprehensive management.
