摘要
运用数据挖掘技术分析东北部地区国医大师刘柏龄教授治疗强直性脊柱炎的学术思想及用药规律。回顾性收集2011-2019年长春中医药大学附属医院刘柏龄教授门诊治疗强直性脊柱炎的病例,运用GraphPad Prism 8、RStudio 4.0.3软件进行数据分析(症状规范、用药统计分析、层次聚类分析、关联规则分析、复杂网络分析)。共纳入处方462首,涉及中药137味,药性以温、寒、平为主,药味以甘、苦为主,主要归经为肝、脾、肾经。高频药物聚类分析,得到6个聚类组;关联规则分析,得到8个常用药对,复杂网络分析显示,核心处方为:附子、肉桂、鸡矢藤、延胡索、鸡血藤、狗脊、杜仲、丹参。刘柏龄教授主张“肾主骨”的学术思想,从“肾督阳虚,外邪侵袭”辨治强直性脊柱炎,以“扶正祛邪,通督壮腰,补益肝肾,祛瘀通络,散寒化湿”为治疗原则,为临床医生提供治疗思路。
The paper aims to analyze the academic thought and medication rules of Professor Liu Bailing,a master of traditional Chinese medicine in Northeast China,in the treatment of ankylosing spondylitis by adopting the data mining technology.In the study,the cases of ankylosing spondylitis treated by Professor Liu Bailing in the affiliated hospital of Changchun University of Chinese Medicine from 2011 to 2019 were retrospectively collected,and the GraphPad Prism 8 and RStudio 4.0.3 softwares were used for data analysis (symptom specification,medication statistical analysis,hierarchical clustering analysis,association rules analysis and complex network analysis).A total of 462 prescriptions were included in the research,involving 137 herbs,which were mainly warm,cold and mild in terms of their natures,while mainly sweet and bitter in terms of their flavors,with their primary channel tropism to the liver,spleen,and kidney meridians.Six cluster groups were obtained by the high-frequency drug clustering analysis,and eight common couplet medicines were obtained by the association rule analysis.While shown by the complex network analysis,the core prescription was made up of the following herbs:prepared common monkshood branched root,cassia bark,Chinese fevervine herb,yanhusuo,suberect spatholobus stem,cibot rhizome,eucommia bark,danshen root.Professor Liu Bailing advocated the academic thought of “kidney governing the bone”,and differentiated and treated ankylosing spondylitis from the perspective of “kidney governing yang de?ciency and invasion of exogenous pathogens”,following the principle of “strengthening vital qi to eliminate pathogenic factors,unblocking the governor channel to strengthen the waist,tonifying the liver and kidney,removing blood stasis and dredging collaterals,dispelling cold and removing dampness”.Professor Liu’s experience and academic thought in this respect are expected to provide some treatment ideas for clinical doctors.
