摘要
目的探讨急性阑尾炎(AA)患者给予腹腔镜阑尾切除术(LA)的疗效。方法选取2019年3月至2022年2月在三门峡市陕州区人民医院接受阑尾切除术的65例AA患者作为研究对象,按照随机数字表法分成观察组(33例)与对照组(32例),对照组给予传统开腹阑尾切除术(OA)治疗,观察组给予LA治疗,比较两组围术期指标、胃肠功能、疼痛程度、炎症因子水平及并发症发生率。结果观察组与对照组相比手术时间、下床活动时间、住院时间更短,差异有统计学意义(P<0.05),术中出血量更少,差异有统计学意义(P<0.05);观察组与对照组相比肛门排气时间、肛门排便时间、肠鸣音恢复时间更短,差异有统计学意义(P<0.05);观察组与对照组相比术后1 h、术后3 h、术后6 h、术后12 h时视觉模拟评分法(VAS)评分更低,差异有统计学意义(P<0.05);两组术后白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、降钙素原(PCT)、C反应蛋白(CRP)水平均提高,差异有统计学意义(P<0.05),但观察组与对照组相比更低,差异有统计学意义(P<0.05);观察组与对照组相比并发症发生率更低,差异有统计学意义(P<0.05)。结论AA患者给予LA治疗,能够改善围术期指标,促进患者胃肠功能恢复,降低疼痛程度,调节炎症因子水平,降低并发症发生率。
Objective To investigate the effect of laparoscopic appendectomy(LA)on acute appendicitis(AA).Methods A total of 65 AA patients who underwent appendectomy in Sanmenxia Shanzhou District People’s Hospital from March 2019 to February 2022 were selected as the study subjects.According to the random number table,they were divided into the observation group(33 cases)and the control group(32 cases).The control group was treated with traditional open appendectomy(OA),and the observation group was treated with LA.The perioperative indicators,gastrointestinal function,pain degree,inflammatory factor level and complication rate of the two groups were compared.Results Compared with the control group,the operation time,ambulatory time and hospitalization time were shorter in the observation group(P<0.05),and the intraoperative bleeding was less(P<0.05);Compared with the control group,the time of anal exhaust,the time of anal defecation and the recovery time of bowel sounds were shorter in the observation group(P<0.05);Compared with the control group,the visual analog score(VAS)of the observation group was lower at 1 h,3 h,6 h and 12 h after operation(P<0.05);The levels of interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),procalcitonin(PCT)and C-reactive protein(CRP)after operation in both groups were significantly increased(P<0.05),but lower in the observation group than those in the control group(P<0.05);The complication rate of the observation group was lower than that of the control group(P<0.05).Conclusion LA therapy in AA patients can improve the perioperative indexes,promote the recovery of gastrointestinal function,reduce the degree of pain,regulate the level of inflammatory factors and reduce the incidence of complications.
