电子支气管镜灌洗联合新鱼腥草素钠静脉滴注治疗支气管扩张合并感染的效果观察 被引量:7

Effect of electronic bronchoscopy lavage combined with intravenous infusion of sodium neohouttuynia on bronchiectasis complicated by infection

摘要 目的探讨电子支气管镜灌洗联合新鱼腥草素钠静脉滴注治疗支气管扩张合并感染的效果。方法抽取枣庄市立医院新城院区2019年7月至2021年9月收治的支气管扩张合并感染患者68例, 按随机数字表法分为对照组(34例)和观察组(34例)。两组患者入院后均采用化痰、抗炎、吸氧等常规治疗方法。对照组患者予以新鱼腥草素钠注射液静脉滴注, 观察组患者在对照组的基础上予以电子支气管镜灌洗, 两组均持续治疗3周。比较两组患者的临床疗效, 治疗前后血气分析指标、调节性免疫指标以及炎症因子指标。结果观察组的临床有效率为94.12%(32/34), 高于对照组的67.65%(23/34), P<0.05。治疗后, 两组动脉血氧分压(PaO2)水平均上升, 且观察组高于对照组(P<0.05);动脉血二氧化碳分压(PaCO_(2))水平均下降, 且观察组低于对照组(P<0.05)。治疗后, 两组血清白细胞介素-6(IL-6)、C反应蛋白(CRP)、降钙素原(PCT)水平均下降, 且观察组低于对照组(P<0.05)。治疗后, 两组辅助性T细胞17(TH17)、TH17/调节性T细胞(Treg)水平均下降, 且观察组低于对照组(P<0.05);Treg水平均上升, 且观察组高于对照组(P<0.05)。结论电子支气管镜灌洗联合新鱼腥草素钠静脉滴注治疗支气管扩张合并感染, 能够通过降低患者的炎症反应, 提高血气指标水平并调节机体免疫反应, 提高治疗效果。 Objective To investigate the effect of electronic bronchoscopy lavage combined with intravenous infusion of sodium neohouttuynia on patients with bronchiectasis complicated by infection.Methods Sixty-eight patients with bronchiectasis complicated by infection admitted to Xincheng District Branch Hospital of Zaozhuang Municipal Hospital from July 2019 to September 2021 were selected and divided into control group(34 cases)and observation group(34 cases)according to random number table method.After admission,patients in the two groups were treated by conventional treatments such as phlegm-reducing,anti-inflammatory and oxygen inhalation.Patients in the control group were given intravenous injection of sodium Houttuynia cordata,and patients in the observation group were given electronic bronchoscopy lavage based on the treatment of the control group.Both groups were treated for 3 weeks continuously.The clinical efficacy,blood gas analysis index,regulatory immune index and inflammatory factor index of the two groups were compared before and after treatment.Results The clinical effective rate of the observation group was 94.12%(32/34),higher than the 67.65%(23/34)of the control group(P<0.05).After treatment,partial pressure of oxygen(PaO2)level was increased in the two groups,and PaO2 in the observation group was higher than that in the control group(P<0.05),while partial pressure of carbon dioxide(PaCO2)level was decreased in the two groups,and PaCO2 in the observation group was lower than that in the control group(P<0.05).After treatment,the levels of serum interleukin-6(IL-6),C-reactive protein(CRP),and procalcitonin(PCT)in the two groups were decreased,and these indexes in the observation group were lower than those in the control group(P<0.05).After treatment,levels of T helper cell 17(TH17)and TH17/regulatory T cell(Treg)in the two groups were decreased,and these indexes in the observation group were lower than those in the control group(P<0.05);while Treg level was increased,and Treg level in the observation group was higher than that in the control group(P<0.05).Conclusions Electronic bronchoscope lavage combined with intravenous drip of sodium neohouttuynia in the treatment of bronchiectasis complicated by infection can reduce the inflammatory response,improve the blood gas index level,regulate the immune response,and improve the treatment effect.
