摘要
目的分析硝苯地平控释片(NCRT)联合替米沙坦片(TT)治疗老年冠心病(CHD)合并顽固性高血压(IH)的效果。方法抽取2018年1月至2020年12月漯河市第六人民医院收治的老年CHD合并IH患者86例,按治疗方法将其分为NCRT组和联合组,每组43例。NCRT组给予NCRT治疗,联合组在NCRT组基础上给予TT治疗,比较两组疗效、舒张压(DBP)和收缩压(SBP)水平、血管内皮功能[内皮素(ET-1)、一氧化氮(NO)]、血清超敏C反应蛋白(hs-CRP)水平及不良反应发生率。结果联合组总有效率(95.35%,41/43)高于NCRT组(74.42%,32/43),P<0.05。治疗后,联合组DBP、SBP水平低于NCRT组(P<0.05);联合组ET-1低于NCRT组,NO高于NCRT组(P<0.05);治疗后,联合组血清hs-CRP低于NCRT组(P<0.05)。联合组不良反应发生率(4.65%,2/43)低于NCRT组(18.60%,8/43),P<0.05。结论NCRT联合TT治疗老年CHD合并IH可提高临床效果,并能降低血压,缓解炎症症状,且不良反应较少。
Objective To analyze the effect of nifedipine controlled release tablets(NCRT)combined with telmisartan tablets(TT)in the treatment of elderly patients with coronary heart disease(CHD)complicated by intractable hypertension(IH).Methods A total of 86 elderly patients with CHD complicated by IH treated in the Sixth People’s Hospital of Luohe from January 2018 to December 2020 were selected,and they were divided into NCRT group and combined group by treatment methods,with 43 patients in each group.The NCRT group was treated with NCRT,and the combined group was treated with TT based on the treatment of the NCRT group.The curative effect,diastolic blood pressure(DBP),systolic blood pressure(SBP),vascular endothelial function assessed by endothelin-1(ET-1)and nitric oxide(NO),serum hypersensitive C-reactive protein(hs-CRP)level and the incidence of adverse reactions were compared between the two groups.Results The total effective rate of the combined group(95.35%,41/43)was higher than that of the NCRT group(74.42%,32/43),P<0.05.After treatment,the levels of DBP and SBP in the combined group were lower than those in the NCRT group(P<0.05).After treatment,the level of ET-1 in the combined group was lower than that in the NCRT group,and the level of NO was higher than that in the NCRT group(P<0.05).After treatment,the serum hs-CRP in the combined group was lower than that in the NCRT group(P<0.05).The incidence of adverse reactions in the combined group was 4.65%(2/43),which was lower than the 18.60%(8/43)in the NCRT group(P<0.05).Conclusions NCRT combined with TT in the treatment of elderly patients with CHD complicated by IH can improve the clinical effect,reduce blood pressure,alleviate inflammatory symptoms,with fewer adverse reactions.
