摘要
目的:探讨“H2H”营养管理模式对胃癌根治病人术后营养状况和生活质量的影响。方法:选取2020年1至10月于东部战区总医院普通外科就诊的84例胃癌病人,随机分为对照组(n=42)和观察组(n=42)。对照组病人采取常规的护理模式,观察组病人在对照组的基础上加用“H2H”营养管理模式即成立专门的营养小组针对中重度营养不良病人,制定合理的营养方案对病人院内-院外进行连续术后的营养指导。比较两组病人的全面主观的自评量表(PG-SGA)血清学的营养相关指标和生活质量评分指标。结果:在术后进行“H2H”营养管理模式干预60 d后,观察组病人PG-SGA的评分显著低于对照组,差异具有统计学意义(P<0.05);白蛋白、前白蛋白、血红蛋白均显著高于对照组,差异均具有统计学意义(P<0.05);观察组生活质量评分各维度均优于对照组,差异均具有统计学意义(P<0.05)。结论:“H2H”营养管理模式可显著改善胃癌根治病人术后病人营养状况,提高其生活质量。
Objective:To explore the effect of"H2H"nutrition management mode on the nutritional status and quality of life of patients with gastric cancer after radical surgery.Methods:A total of 84 gastric cancer patients who were treated in our hospital from January to October 2020 were enrolled and randomly divided into control group(n=42)and observation group(n=42).The patients in the control group adopt the conventional nursing model,and the patients in the observation group use the"H2H"nutrition management model on the basis of the control group,which means that a special nutrition team will be established for patients with moderate to severe malnutrition,and a reasonable nutrition plan will be developed for the patients in hospital and out of hospital.Nutritional guidance was implemented after continuous operation.The comprehensive and subjective self-rating scale(PG-SGA)serological nutrition-related indicators and quality of life scoring indicators of the two groups of patients were compared.Results:After 60 days of intervention in the"H2H"nutritional management mode,the PG-SGA scores of the observation group were significantly lower than those of the control group,and the difference was statistically significant(P<0.05).Albumin(ALB),prealbumin Protein(prealbumin,PA)and hemoglobin(hemoglobin,HB)were significantly higher than those of the control group,and the differences were statistically significant(P<0.05).The quality of life of the observation group were better than those of the control group in all dimensions,and the differences were statistically significant(P<0.05).Conclusion:The"H2H"nutritional management model can improve the nutritional status of patients after radical gastric cancer surgery and improve their quality of life.
