The Research on the Clinical Diagnostic Value of FPG,HbA1c,Sur,Scr,CysC and C1q for the Early Diabetic Kidney Disease

摘要 目的探讨FPG、HbA1c、Sur、Scr、CysC、C1q对早期糖尿病肾病的诊断价值。方法选取本院2019年9月至2021年5月收治的2型糖尿病患者212例,按照尿白蛋白肌酐比值(UACR)分为UACR正常组(UACR<30mg/g,134例)和UACR轻度增高组(30 mg/g≤UACR≤299 mg/g,78例)。分别进行FPG、HbA1c、Sur、Scr、CysC、C1q的检测,并对结果进行分析,评价各指标对早期糖尿病肾病的诊断价值。结果与UACR正常组相比,UACR轻度增高组FPG、HbA1c、Sur、CysC、C1q水平均增高,差异具有统计学意义(P<0.05)。ROC曲线显示,5项联合检测(AUC:0.756,灵敏度:73.08%,特异性:66.42%)优于FPG(AUC:0.642,灵敏度:67.95%,特异性:56.72%)、HbA1c(AUC:0.585,灵敏度:57.69%,特异性:58.96%)、Sur(AUC:0.583,灵敏度:66.67%,特异性:51.49%)、CysC(AUC:0.678,灵敏度:56.41%,特异性:76.12%)和C1q(AUC:0.587,灵敏度:47.44%,特异性:75.37%)单独检测,但与FBG+CysC联合检测(AUC:0.750,灵敏度:80.77%,特异性:62.69%)相比,差异无统计学意义(P>0.05)。结论FPG、HbA1c、Sur、CysC、C1q对早期糖尿病肾病的诊断具有一定参考价值,联合检测能提高诊断效能,但5项指标联合检测诊断效能与FBG+CysC联合检测差异无统计学意义,而FBG+CysC联合检测更利于临床推广。 Objective To explore the clinical diagnostic value of FPG,HbA1c,Sur,Scr,CysC and C1q for the early diabetic kidney disease.Methods We selected 212 diabetic patients from September,2019 to May,2021 in our hospital.According to urine albumin creatine ratio(UACR),these diabetic patients were divided into the normal UACR group(UACR<30mg/g,134 case)and the slightly increased UACR group(30 mg/g≤UACR≤299 mg/g,78 case).Levels of FPG,HbA1c,Sur,Scr,CysC and C1q of all cases were tested.The clinical diagnostic values for early diabetic kidney disease of single or combined tests were estimated.Results Compared with the normal UACR group,levels of FPG,HbA1c,Sur,CysC and C1q in the slightly increased UACR group were significantly higher(P<0.05).Receiver operating characteristic curve showed that the combined detection of 5 indicators(AUC:0.756,sensitivity:73.08%,specificity:66.42%)was better than FPG(AUC:0.642,sensitivity:67.95%,specificity:56.72%),HbA1c(AUC:0.585,sensitivity:57.69%,specificity:58.96%),sur(AUC:0.583,sensitivity:66.67%,specificity:51.49%)CysC(AUC:0.678,sensitivity:56.41%,specificity:76.12%)and C1q(AUC:0.587,sensitivity:47.44%,specificity:75.37%)detected individually.But compared with FBG+CysC,no significant difference was observed(P>0.05).Conclusion The detection of serum FPG,HbA1c,Sur,CysC and C1q has a certain reference value in the diagnosis of the early diabetic kidney disease.The combined detection of these five indicators has a better diagnostic performance,but the combined detection of FBG+CysC is more conducive for clinical applications.
