微创经皮钢板接骨术辅助LCP内固定治疗胫骨远端骨折对患者膝关节活动度及HSS评分的影响 被引量:23

Effect of minimally invasive percutaneous plate osteosynthesis combined with LCP internal fixation for treating distal tibial fractures on knee range of motion and HSS score

摘要 目的:研究微创经皮钢板接骨术(MIPPO)辅助锁定加压钢板(LCP)内固定治疗胫骨远端骨折对患者膝关节活动度及膝关节功能评分(HSS)评分的影响.方法:选取2018年3月~2020年12月本院收治的胫骨远端骨折患者84例,按照治疗方法不同分为对照组43例(行切开复位内固定术)与观察组41例(行MIPPO辅助LCP内固定术).比较两组患者骨折愈合时间、住院时间、术中出血量、手术时间、膝关节屈伸最大活动度、HSS评分、Maryland足功能评分及并发症发生情况.结果:观察组患者骨折愈合时间、住院时间及手术时间均比对照组短,术中出血量比对照组少;观察组出院后6个月膝关节屈伸最大活动度均大于对照组高;观察组出院后6个月HSS评分、Maryland足功能评分均高于对照组;观察组并发症总发生率低于对照组.结论:微创经皮钢板接骨术辅助LCP内固定治疗胫骨远端骨折可以有效改善患者膝关节活动度,提高HSS评分,促进骨愈合. Objective To study the effect of minimally invasive percutaneous plate osteosynthesis(MIPPO)combined with locking compression plate(LCP)internal fixation for treating distal tibial fractures on knee range of motion and hospital for spe-cial surgery knee score(HSS score).Methods A total of 84 patients with distal tibial fractures admitted to the hospital between Mar 2018 and December 2020 were selected.They were divided into control group(43 cases,open reduction and internal fixa-tion)and observation group(41 cases,MIPPO combined with LCP internal fixation)according to the surgical method.Fracture healing time,length of hospital stay,intraoperative blood loss,operation time,maximum range of knee flexion and extension,HSS score,Maryland foot function score,and complications were compared between the two groups.Results The fracture heal-ing time,hospital stay,operation time and intraoperative blood loss of observation group were shorter/less than those of the con-trol group.The maximum range of knee flexion and extension of the observation group was greater than that of the control group at 6 months after discharge.The HSS score and Maryland foot function score of observation group were higher than those of the control group at 6 months after discharge.The total incidence of complications in the observation group was lower than that in the control group.Conclusion MIPPO combined with LCP internal fixation for treating distal tibial fractures can effectively improve knee range of motion,increase the HSS score,and promote bone healing.
