摘要
目的探讨氟哌噻吨美利曲辛联合正念认知疗法治疗卒中后抑郁的临床疗效及安全性.方法选取2019年1月-2021年1月天津市静海区医院神经内科住院患者80例卒中后抑郁患者,分为观察组和对照组各40例.两组均给予脑卒中的基础治疗,观察组同时予以氟哌噻吨美利曲辛早1片、三天后加量至早、中各1片口服.正念心理认知治疗1-2次/周,共8周.分析比较两组治疗后临床疗效及HAMD和NHISS评分有无差异.结果治疗8周后观察组总有效高于对照组,有统计学差异(P<0.05).两组治疗过程中均无明显不良反应,观察组有5例患者出现轻微的失眠、不安、躁动、头晕和震颤、口感、便秘等胃肠道反应,停药后消失,无一例患者因不能耐受而需停药.治疗后观察组和对照组HAMD评分分别为(8.23±2.43)和(10.20±2.07),NHISS评分分别为(10.33±2.65)和(16.23±3.76),较治疗前均明显降低(P<0.05).但观察组治疗后显著低于对照组,且观察组评分下降更为迅速(P<0.05).结论氟哌噻吨美利曲辛联合正念认知疗法治疗卒中后抑郁疗效确切,可短期内改善患者抑郁症状和神经功能评分.
Objective To investigate the clinical efficacy and safety of Flupentixol and melitracen combined with mindfulness cognitive therapy in the treatment of post-stroke depression.Methods A total of 80 patients with post-stroke depression in the Department of Neurology of our hospital from January 2019 to January 2021 were selected and divided into observation group and control group,with 40 cases in each group.Both groups were given the basic treatment of stroke.The observation group was given Flupentixol and melitracen as early as 1 tablet,and one tablet in the morning and middle three days later.Mindfulness psychological cognitive therapy 1-2 times/week,a total of 8 weeks.The clinical efficacy and HAMD and nhiss scores of the two groups were analyzed and compared.Results After 8 weeks of treatment,the total effective rate of the observation group was higher than that of the control group(P<0.05).There were no obvious adverse reactions during the two treatments.In the treatment group,5 patients had slight gastrointestinal reactions such as insomnia,restless ness,agitation,dizziness and tremor,taste and constipation,which disappeared after stopping the drug,and no patient needed to stop the drug because of intolerance.After treatment,the HAMD scores of the observation group and the control group were(8.23±2.43)and(10.20±2.07)respectively,and the nhiss scores were(10.33±2.65)and(16.23±3.76)respectively,which were significantly lower than those before treatment(P<0.05).However,the score of the observation group was significantly lower than that of the control group after treatment,and the score of the observation group decreased more rapidly(P<0.05).Coriclusion Flupentixol melitracen combined with mindfulness cognitive therapy is effective in the treatment of post-stroke depression,and can improve the depressive symptoms and neurological function scores in a short time.
