miR-126、Lp-PLA2和TNF-α在急性脑梗死中的表达及临床意义 被引量:7

Expression and clinical significance of miR-126,Lp-PLA2 and TNF-α in acute cerebral infarction

摘要 目的 分析微小RNA-126(miR-126)、脂蛋白磷脂酶A2(Lp-PLA2)、肿瘤坏死因子(TNF-α)在急性脑梗死(ACI)中的表达及临床意义。方法 选取2019年9月至2021年9月漯河市第三人民医院收治的119例ACI患者纳入ACI组,根据不同梗死面积将患者分为小面积组、中面积梗死组、大面积组,根据不同神经缺损程度将患者分为轻度组、中度组、重度组,根据不同预后将患者分为生存组、死亡组。另选取同期于本院进行健康体检的62例健康者作为对照组。比较ACI组和对照组以及ACI组不同梗死面积、神经缺损程度、预后结局患者miR-126、Lp-PLA2、TNF-α水平。结果 ACI组miR-126水平明显低于对照组,Lp-PLA2、TNF-α水平明显高于对照组,差异均有统计学意义(P<0.05)。小面积梗死者46例(小面积组),中面积梗死者35例(中面积组),大面积梗死者38例(大面积组)。不同梗死面积患者miR-126水平比较:小面积组>中面积组>大面积组,Lp-PLA2、TNF-α水平比较:小面积组<中面积组<大面积组,差异均有统计学意义(P<0.05)。神经功能轻度缺损者49例(轻度组),中度缺损者35例(中度梗死组),重度缺损者35例(重度组)。不同神经功能缺损程度患者miR-126水平比较:轻度组>中度组>重度组,Lp-PLA2、TNF-α水平比较:轻度组<中度组<重度组(P<0.05),差异均有统计学意义。预后生存者99例(生存组),死亡者20例(死亡组)。生存组miR-126水平明显高于死亡组,Lp-PLA2、TNF-α水平低于死亡组,差异均有统计学意义(P<0.05)。结论 ACI患者血清miR-126、Lp-PLA2、TNF-α水平存在异常表达,且与患者梗死面积、神经功能缺损程度及预后存在紧密联系,可作为病情监测、预后评估的辅助指标。 Objective To analyze the expression and clinical significance of microRNA-126(miR-126),lipoprotein phospholipase A2(Lp-PLA2),tumor necrosis factor-α(TNF-α)in acute cerebral infarction(ACI). Methods 119 ACI patients admitted to the Third People’s Hospital of Luohe from September2019 to September 2021 were selected and included in the ACI group,according to different infarct sizes,patients were divided into the small area group,the medium area infarction group and the large area group,and according to different nerve defect degrees,patients were divided into the mild group,the moderate infarction group and the severe group,according to different prognosis,patients were divided into the survival group and the death group. In addition,62 healthy people who underwent physical examination in our hospital during the same period were selected as the control group. The levels of miR-126,Lp-PLA2,and TNF-α in the ACI group,the control group and the ACI group with different infarct size,nerve defect degree and prognosis were compared. Results The level of miR-126 in the ACI group was significantly lower than that in the control group,and the levels of Lp-PLA2 and TNF-α were significantly higher than those in the control group,the differences were all statistically significant(P<0.05). There were 46 cases of small area infarction(small area group),35 cases of medium area infarction(medium area group)and 38 cases of large area infarction(large area group). Comparison of miR-126 levels in patients with different infarct sizes:small area group > medium area group > large area group,comparison of Lp-PLA2 and TNF-α levels:small area group < medium area group < large area group,the difference was statistically significant(P<0.05). There were 49 cases of mild neurological impairment(mild group),35 cases of moderate neurological impairment(moderate infarction group)and 35 cases of severe neurological impairment(severe group). Comparison of miR-126 levels in patients with different neurological deficits:mild group > moderate group > severe group,comparison of Lp-PLA2 and TNF-α levels:mild group < moderate group < severe group(P<0.05),99 cases were survivors(survival group)and20 cases died(death group). The level of miR-126 in the survival group was significantly higher than that in the death group,the levels of Lp-PLA2 and TNF-α were lower than those in the death group,and the difference was statistically significant(P<0.05). Conclusion The serum levels of miR-126,Lp-PLA2 and TNF-α are abnormally expressed in ACI patients,and they are closely related to the infarct size,neurological deficit and prognosis. They can be used as auxiliary indicators for disease monitoring and prognostic evaluation.
