摘要
目的:分析广东省公立医院患者就医流向及医疗费用变化情况,评估分级诊疗政策实施效果,提出相关政策建议。方法:对2016—2019年广东省不同地区、不同级别医院门急诊人次、出院人数、医疗费用等数据进行比较分析。结果:广东省公立医院患者就医需求趋高,主要集中在三级医院;分级诊疗的控费预期效果未达,医疗费用总体增长较快;虽然均次费用增速减慢,但仍需建立长效控费机制。结论:需优化资源配置,提高基层医疗服务能力,大力发展医共体/医联体,发挥医保支付杠杆作用,合理分流患者,推进分级诊疗。
Objective To explore the trend of patient visits and medical expenses in different types of public hospitals in Guangdong Province in the context of hierarchical medical system.Methods The data of outpatient and emergency inpatients'medical expenses in hospitals of different grades in different regions of Guangdong Province from 2016 to 2019 were compared and analyzed.Results Public hospitals in the Pearl River Delta were still the main providers of medical and health services in Guangdong Province.The proportion of service quantity and total medical expenses in tertiary public hospitals was the highest,and showed an increasing trend year by year.Conclusions The implementation of hierarchical medical system has not reached the expected goal.It is still necessary to optimize resource allocation and deepen the construction of medical union mode,give play to the leverage role of medical insurance payment,promote rational medical treatment of patients,and strengthen cost control.
