摘要
目的探讨坦索罗辛联合艾灸刺激胫神经相关穴位对老年前列腺增生围手术期膀胱过度活动症(OAB)的临床疗效。方法将54例老年前列腺增生患者随机分为两组(各27例),对照组入组后开始服用盐酸坦索罗辛缓释胶囊,至行经尿道等离子前列腺剜除术后7天,共15天。观察组在对照组的基础上同时加行艾灸盒灸委中穴、关元穴。比较两组术后国际前列腺症状评分(IPSS)、生活质量(QoL)评分、膀胱过度活动症评分(OABSS)、最大尿流率(Q_(max))、残余尿量(PVR)及术后膀胱痉挛疼痛发生率。结果两组术前IPSS、QoL、OABSS对比无显著差异(P>0.05),但两组内术后较术前改善明显,具有显著统计学差异(P<0.05)。两组间比较,观察组术后明显低于对照组,差异具有统计学意义(P<0.05)。膀胱痉挛疼痛发生率观察组术后低于对照组,差异具有统计学意义(P<0.05)。两组Q max、PVR术前比较无显著差异(P>0.05),但两组内术后较术前改善明显,具有显著统计学差异(P<0.05)。两组间比较,术后Q max、PVR比较无显著差异(P>0.05)。结论坦索罗辛联合艾灸刺激胫神经相关穴位,能更好地缓解老年男性前列腺增生围手术期OAB症状,体现中医外治法之优势,建议临床选择使用。
OBJECTIVE To investigate the clinical effect of tamsulosin capsules combined with moxibustion to stimulate tibial nerve related acupoints on perioperative overactive bladder(OAB)syndrome in elderly patients with benign prostatic hyperplasia.METHODS 54 elderly patients with benign prostatic hyperplasia were randomly divided into two groups(27 cases in each group).The control group was treated from preoperative with tamsulosin hydrochloride sustained-release capsules until 7 days after transurethral plasmakinetic enucleation of the prostate,a total of 15 days after admission.In the observation group,weizhong point and Guanyuan point of moxibustion boxes were added simultaneously on the basis of the control group.International prostate symptom Score(IPSS),quality of life(QoL)score,overactive bladder syndrome score(OABSS),maximum uroflowmetry(Q max),postvoidresidual volume(PVR)and incidenceof cystospasm were compared between the two groups.RESULTS There was no significant difference in preoperative IPSS,QoL and OABSS within the two groups(P>0.05),but the postoperative improvement was significantly better than that before operation in the two groups,with a significant statistical difference(P<0.05).Compared between the two groups,the observation group was significantly lower than the control group,the difference was statistically significant(P<0.05).The incidence of cystospasmafter operation in observation group was significantly lower than that in control group(P<0.05).There was no significant difference in Q_(max) and PVR between the two groups before operation(P>0.05),but there was significant difference within the two groups after operation(P<0.05).There was no significant difference in Q max and PVR between the two groupsafter operation(P>0.05).CONCLUSION Tamsulosin capsules combined with moxibustion to stimulate tibial nerve related acupoints can better relieve OAB symptoms in elderly men with benign prostatic hyperplasia during operation,which reflects the advantages of external treatment of traditional Chinese medicine and suggests clinical use.
