摘要
目的观察委中穴放血治疗瘀血型非特异性下腰痛的临床疗效。方法将120例瘀血型非特异性下腰痛患者随机分为放血组与针刺组、西药组3组,放血组在患者双侧委中穴及附近青紫脉络放血并拔罐,针刺组常规针刺双侧肾俞、膈俞、大肠俞、阿是穴、委中穴西药组口服右旋布洛芬,隔日1次,1周为1个疗程,3个疗程后观察视觉模拟评分(VAS)、腰背及下肢功能障碍评分(ODI)、坦帕运动恐惧症量表得分(TKS)、诺丁汉健康量表得分(NHP)、6min步行试验(6MWT)、白介素-1(IL-1)、前列腺素2(PGE2)及临床疗效。结果治疗后,放血组和针刺组、西药组的VAS评分、ODI评分、TKS评分、NHP评分、IL-1、PGE2表达均明显降低,6MWT升高与治疗前比较差异有统计学意义(P<0.05);放血组总有效率优于针刺组和西药组(P<0.05),针刺组和西药组比较无统计学差异(P>0.05)。结论放血治疗瘀血型非特异性下腰痛疗效较好,值得临床推广。
Objective To observe the clinical effect of blood-letting at Weizhong points in treatment of blood stasis type of non-specific low back pain.Methods 120 cases of patients with blood stasis type of non-specific low back pain were randomly divided into blood-letting group,acupuncture group and western medicine group.Blood-letting group was treated with blood-letting and cupping at bilateral Weizhong points and nearby blue and purple veins;acupuncture group was treated with routinely acupuncture at Shenshu,Geshu,Dachangshu Ashi and Weizhong points;western medicine group was treated with dexibuprofen once every other day,once a week for a course of treatment.Visual Analogue Score(VAS),lumbar back and lower limb dysfunction score(OPI)Tampa Kinesiophobia Scale Score(TKS),Nottingham Health Profile(MPH)Score(NHP),6-min walk test(6 MWT),the levels of interleukin-1(IL-1)and prostaglandin E2(PGE2)were observed after 3 courses of treatment.Results After treatment,the VAS,ODI,TKS,NHP,MWT,scores the IL-1 and PGE2 expressions of all groups were significantly lower than those before treatment(P<0.05),which were significantly higher than those before treatment(P<0.05);the total effective rate of blood-letting group was was better than acupuncture group and medicine group(P<0.05),there was no significant difference between acupuncture group and medicine group(P>0.05).Conclusion Blood-letting is effective in treatment of blood stasis type of non-specific low back pain,which is worthy of clinical promotion.
