摘要
目的探讨综合营养干预对骨质疏松性骨折患者营养状态及运动功能的影响。方法抽取2019年2月至2020年12月在河南省人民医院骨科治疗的114例骨质疏松性骨折患者,采用随机数字表法将其分为对照组和研究组,每组57例。两组患者均接受手术治疗和抗骨质疏松常规治疗,对照组住院期间接受常规康复护理,研究组在对照组基础上给予综合营养干预。比较两组干预前及干预6个月后营养指标水平、第2~4腰椎侧位骨密度,血钙、血磷水平及运动功能[Fugl-Meyer评估量表(FMA)评分]。结果干预6个月后,两组血红蛋白、血清总蛋白及白蛋白水平均高于干预前,且研究组高于对照组(P<0.05);研究组血钙水平为(2.50±0.27)mmol/L,第2~4腰椎侧位骨密度为(0.59±0.07)g/cm^(2),高于干预前[(1.91±0.20)mmol/L、(0.56±0.09)g/cm^(2)]和对照组[(1.99±0.26)mmol/L、(0.54±0.11)g/cm^(2)],P均<0.05;两组患者上肢运动功能、下肢运动功能评分及FMA总分均高于干预前,且研究组高于对照组(P<0.05)。结论综合营养干预可改善骨质疏松性骨折患者术后营养状态,有效增加患者血钙水平和骨密度,还可有效改善患者术后运动功能。
Objective To investigate the effect of comprehensive nutrition intervention on nutritional status and motor function of patients with osteoporotic fractures.Methods A total of 114 patients with osteoporotic fractures treated in the Department of Orthopedics of Henan Provincial People’s Hospital from February 2019 to December 2020 were selected and divided into control group and study group by random number table method,with 57 cases in each group.All patients received surgical treatment and conventional anti-osteoporosis treatment,the control group received conventional rehabilitation care during hospitalization,and the study group received comprehensive nutritional intervention based on the treatment of the control group.The nutritional index levels,lateral bone density of the 2nd lumbar vertebra to the 4th lumbra vertebra(L2-4),levels of blood calcium,blood phosphorus and motor function(Fugl-Meyer assesment score)before intervention and after 6 months of intervention were compared between the two groups.Results After 6 months of intervention,the levels of hemoglobin,total serum protein and albumin in the two groups were higher than those before intervention,and those in the study group were higher than those in the control group(P<0.05);the level of blood calcium in the study group was(2.50±0.27)mmol/L,the lateral bone density of the L2-4 was(0.59±0.07)g/cm^(2),which were higher than the(1.91±0.20)mmol/L and(0.56±0.09)g/cm^(2) before intervention,and higher than the(1.99±0.26)mmol/L and(0.54±0.11)g/cm^(2) in the control group(all P<0.05);the scores of upper limb motor function,lower limb motor function and Fugl-Meyer assesment total score in the two groups were higher than those before intervention,and those in the study group were higher than those in the control group(P<0.05).Conclusions Comprehensive nutritional intervention can improve postoperative nutritional status of patients with osteoporotic fracture,effectively increase blood calcium level and bone mineral density,and effectively improve postoperative motor function.
