摘要
目的:通过高频彩超观察腹腔镜下不同术式治疗精索静脉曲张(VC)的疗效及血流动力学改变。方法:选取泌尿外科住院治疗的100例精索静脉曲张患者,随机均分为观察组及对照组,观察组采用腹腔镜下精索血管集束状结扎术(Palomo术式),对照组采用腹腔镜单纯精索内静脉结扎术(Ivanissevich术式),并通过高频彩超比较两组患者术前及术后1个月的精索静脉最大内径、睾丸体积和睾丸动脉血流动力学变化[收缩期最大血流速度(V_(max))、平均血流速度(V_(mean))以及舒张期最小血流速度(V_(min))]。结果:两组患者术后精索静脉最大内径均较术前减小,其中观察组[(1.50±0.22)mm]明显小于对照组[(1.89±0.24)mm],且差异有统计学意义(P<0.05);两组患者睾丸体积组间、组内,差异均无统计学意义(P>0.05);两组患者睾丸动脉V_(max)、V_(mean)、V_(min)组间比较、组内比较,差异均无统计学意义(P>0.05)。结论:精索静脉曲张患者腹腔镜手术无论选用Palomo术式或Ivanissevich术式均具有安全性,但Palomo术式手术效果更为明显。
Objective To observe the curative effect and hemodynamic changes of different surgical methods for varicocele under laparoscopy through high-frequency color Doppler ultrasound.Method A total of 100 patients with varicocele who were hospitalized in the Department of Urology in our hospital were selected and randomly divided into observation group and control group.The observation group underwent laparoscopic cluster ligation(Palomo technique),and the control group underwent laparoscopic high ligation of spermatic vein(Ivanissevich technique).The maximum diameter of spermatic vein,testicular volume and systolic hemodynamic changes of testicular artery(V_(max),V_(mean) and V_(min))were compared between 2 groups before and 1 month after operation by high-frequency color ultrasound.Results The maximum inner diameter of the spermatic vein in the two groups was reduced compared with that before the operation.The observation group[(1.50±0.22)mm]was significantly smaller than the control group[(1.89±0.24)mm],and the difference was statistically significant(P<0.05);There was no significant difference in testicular volume between and within the two groups(P>0.05);There was no significant difference in testicular artery V_(max),vmean and V_(min) between and within the two groups(P>0.05).Conclusion Laparoscopic surgery for varicocele patients is safe regardless of whether Palomo technique or Ivanissevich technique is used,but the Palomo surgery is more effective.
