摘要
目的探讨双切口囊外白内障摘除术(ECCE)治疗硬核白内障患者的效果。方法选取2020年2月至2021年2月河南省南阳市中心医院收治的硬核白内障患者95例,按照随机数字表法分为对照组(N=47)和研究组(N=48),对照组采用单切口ECCE治疗,研究组采用双切口ECCE治疗。治疗3周后评估患者疗效,对比分析两组的眼压、散光度数及6周后的矫正视力,统计并发症情况。结果治疗3周后,研究组治疗有效率(95.83%)高于对照组(82.98%),差异有统计学意义(P<0.05);两组治疗3周后眼压和散光度数均低于治疗前,且研究组均低于对照组,差异有统计学意义(P<0.05);研究组治疗6周后矫正视力为(0.43±0.15),对照组矫正视力为(0.35±0.14),研究组较对照组高,差异有统计学意义(P<0.05);对照组发生前房充血3例,眼内炎4例,角膜水肿2例,研究组发生眼内炎1例,角膜水肿1例,研究组的并发症发生率(4.17%)低于对照组的(19.15%),差异有统计学意义(P<0.05)。结论双切口ECCE治疗硬核白内障患者效果显著,不仅可以降低患者的眼压及散光度数,且能矫正视力,降低并发症的发生。
Objective To investigate the effect of double incision extracapsular cataract extraction(ECCE)in the treatment of patients with hard nuclear cataract.Methods A total of 95 patients(95 eyes)with hard nuclear cataract admitted to our hospital from February 2020 to February 2021 were selected and divided into control group(N=47)and study group(N=48)according to the random number table method.The control group was treated with single-incision ECCE,and the study group was treated with double-incision ECCE.The curative effect of the patients was evaluated after 3 weeks of treatment,the intraocular pressure,astigmatism,and corrected visual acuity after 6 weeks were compared between the two groups,and the complications were counted.Results The total effective rate of the study group was 95.83%higher than that of the control group,82.98%,after 3 weeks of treatment(P<0.05).The intraocular pressure and astigmatism in the two groups were lower than those before treatment,and the research group was lower than the control group after 3 weeks of treatment(P<0.05).After 6 weeks of treatment,the corrected visual acuity of the study group was(0.43±0.15),and the corrected visual acuity of the control group was(0.35±0.14),which was lower than that in the study group(P<0.05).There were 3 cases of anterior chamber congestion,4 cases of endophthalmitis,and 2 cases of corneal edema in the control group,while 1 case of endophthalmitis and 1 case of corneal edema occurred in the study group.The total incidence of complications in the study group was 4.17%,lower than that in the control group,which was 19.15%(P<0.05).Conclusion Double-incision ECCE has a remarkable effect in the treatment of hard nuclear cataract patients,which can not only reduce the patient's intraocular pressure and astigmatism,but also correct visual acuity and reduce the occurrence of complications.
