摘要
目的探讨白内障术后干眼症患者联合应用人工泪液与明目地黄丸治疗的临床效果。方法选取该院2022年7月—2023年7月进行超声乳化手术治疗后出现干眼症的180例白内障患者为研究对象,根据随机数字表法将其分为对照组与观察组,各90例。对照组采用单用人工泪液治疗,观察组联合应用人工泪液与明目地黄丸治疗。对比两组患者的治疗效果、症状评分、泪液功能、眼压、眼动脉血流动力学及不良反应发生情况。结果观察组治疗总有效率为92.22%,高于对照组的80.00%,差异有统计学意义(P<0.05)。治疗前,两组主观症状评分、客观症状评分比较,组间差异无统计学意义(P>0.05);治疗3个月后,观察组主观症状评分为(1.64±0.27)分、客观症状评分为(1.11±0.21)分,均低于对照组的(2.38±0.22)分、(1.95±0.23)分,组间差异有统计学意义(P<0.05)。治疗前,两组泪膜破裂时间(BUT)、泪液分泌试验(SIt)比较,组间差异无统计学意义(P>0.05);治疗3个月后,观察组BUT为(12.21±1.03)s、SIt为(15.05±1.28)mm,均长于对照组的(9.98±1.05)s、(11.37±0.61)mm,组间差异有统计学意义(P<0.05)。治疗前,两组眼压、眼动脉阻力指数(RI)、眼动脉平均流速(Vm)比较,差异无统计学意义(P>0.05);治疗3个月后,观察组眼压为(12.49±2.06)mmHg(1 mmHg=0.133 kPa)、RI为(0.49±0.01),均低于对照组的(15.95±1.99)mmHg、(0.70±0.02),Vm为(4.48±0.31)cm/s,快于对照组的(3.11±0.32)cm/s,组间差异有统计学意义(P<0.05)。两组不良反应总发生率比较,差异无统计学意义(P>0.05)。结论白内障患者行超声乳化手术后应用人工泪液与明目地黄丸治疗干眼症具有显著效果,可减轻眼部症状,改善眼睛泪液功能,降低眼压。
Objective To observe and explore the clinical effect of combined use of artificial tears and Mingmu Dihuang Pills on postoperative dry eye syndrome patients after cataract surgery.Methods 180 cataract patients who developed dry eye after phacoemulsification surgery in our hospital from July 2022 to July 2023 were selected as the study objects,and were divided into a control group and an observation group according to random number table method,with 90 cases in each group.The control group was treated with artificial tears alone,and the observation group was treated with artificial tears combined with Mingmu Dihuang pill.The therapeutic effect,symptom score,tear function,intraocular pressure,ocular arterial hemodynamics and the occurrence of adverse reactions were compared between the two groups.Results The total effective rate of the observation group was 92.22%,higher than 80.00%of the control group,and the difference was statistically significant(P<0.05).Before treatment,there were no significant differences between the two groups in subjective symptom score and objective symptom score(P>0.05);after 3 months of treatment,the subjective symptom score and objective symptom score of the observation group were(1.64±0.27)points and(1.11±0.21)points,both of which were lower than(2.38±0.22)points and(1.95±0.23)points of the control group,and the differences between groups were statistically significant(P<0.05).Before treatment,there were no significant differences in tear film rupture time(BUT)and tear secretion test(SIt)between the two groups(P>0.05);after 3 months of treatment,BUT and SIt in the observation group were(12.21±1.03)s and(15.05±1.28)mm,both of which were longer than(9.98±1.05)s and(11.37±0.61)mm in the control group,with statistical significance(P<0.05).Before treatment,there were no significant differences in IOP,ocular arterial resistance index(RI)and average flow velocity(Vm)between the two groups(P>0.05),after 3 months of treatment,the IOP and RI in the observation group were(12.49±2.06)mmHg(1 mmHg=0.133 kPa)and(0.49±0.01),both lower than(15.95±1.99)mmHg and(0.70±0.02)in the control group,and Vm were(4.48±0.31)cm/s,it was faster than(3.11±0.32)cm/s in the control group,and the differences between groups were statistically significant(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion The use of artificial tears and Mingmu Dihuang pills in the treatment of dry eye after phacoemulsification surgery for cataract patients has a significant effect,which can alleviate eye symptoms,improve tear function,and reduce intraocular pressure.
