营养支持对老年髋部骨折患者围手术期营养状况及预后的影响 被引量:17

摘要 目的:探究营养支持对老年髋部骨折患者围手术期营养状况和预后的影响。方法:收集2019年1—12月在我院手术治疗的200例老年髋部骨折患者为研究对象。根据术前MNA评分,随机分为观察组和对照组各100例患者,两组患者一般情况无差异(P>0.05)。两组患者均行常规治疗方案手术治疗、常规术后护理及膳食指导,观察组患者在此基础上给予口服营养补充。比较两组患者围手术期营养状态及术后3月预后情况。结果:2组患者入院时血清ALB、PA、HGB及Lc均无差异(P>0.05),术后第1天观察组患者血清PA和Lc水平下降程度低于对照组患者(P<0.05),术后第7天,观察组患者血清ALB、PA、HGB及Lc均高于对照组(P<0.05)。观察组患者在术后3月内并发症发生率显著低于对照组(P<0.05),平均伤口愈合时间和平均住院时长均显著短于对照组(P<0.05),术后3个月ADL评分显著高于对照组(P<0.05)。结论:对老年髋部骨折患者在围手术期及康复期进行营养支持可显著改善患者围手术期营养状况,缩短伤口愈合时间和住院时间,降低并发症发生率,提高ADL评分,改善预后。 Objective To explore the effect of nutritional support on the perioperative nutritional status and prognosis of elderly patients with hip fracture.Method A collection of 200 geriatric hip fracture patients who were surgically treated in our hospital from January to December in 2019 as the research object.According to the preoperative MNA score,they were randomly divided into the observation group and the control group with 100 patients each.There was no difference in general conditions between the two groups(P>0.05).Patients in the two groups underwent conventional treatment programs for surgical treatment,conventional postoperative care,and nutritional guidance.On this basis,patients in the observation group were given oral nutritional supplements.The nutritional status of the two groups during the perioperative period and the prognosis of 3 months after the operation were compared.Result There was no difference in serum ALB,PA,HGB and Lc between the two groups of patients on admission(P>0.05).The level of serum PA and Lc in the observation group was lower than that in the control group on the first day after surgery(P<0.05).On the 7th day after the operation,the serum ALB,PA,HGB and Lc in the observation group were all higher than those in the control group(P<0.05).The incidence of complications in the observation group within 3 months after surgery was significantly lower than that in the control group(P<0.05),and the average wound healing time and average length of hospital stay were significantly shorter than those in the control group(P<0.05).The ADL score at 3 months after operation was significantly higher than that of the control group(P<0.05).Conclusion Nutritional support for elderly patients with hip fracture during the perioperative and rehabilitation period can significantly improve the patient's perioperative nutritional status,shorten the wound healing time and hospital stay,reduce the incidence of complications,increase the ADL score,and improve the prognosis.
