摘要
目的:系统评价中药干预帕金森病伴睡眠障碍患者的疗效。方法:全面检索中英文数据库中涉及帕金森病伴睡眠障碍中医或中西医相关的临床随机对照试验研究,检索时间为建库至2021年8月,应用Rev Man 5.4.1软件进行数据分析。结果:共纳入文献23篇,样本量为1483例。Meta分析结果示与单纯西药治疗比较,中药组的以下指标水平均更优:临床疗效(P<0.00001)、中医证候积分(P<0.00001)、匹兹堡睡眠质量指数量表(Pittsburgh Sleep Quality Index,PSQI)评分(P<0.00001)、统一帕金森病评定量表第三部分(Unified Parkinson’s Disease Rating Scale Part III,UPDRSⅢ)评分(P<0.00001)、帕金森病睡眠量表(Parkinson’s Disease Sleep Scale,PDSS)评分(P<0.00001)、爱泼沃斯嗜睡量表(Epworth Sleepiness Scale,ESS)评分(P=0.02)、39项帕金森病生存质量调查问卷(The 39-item Parkinson’s Disease Quality of Life Questionnaire,PDQ-39)评分(P<0.00001)。结论:中药干预较单纯使用西药可更好地改善帕金森病患者的睡眠障碍,且不良反应的报道较少。
Objective:To systematically evaluate the efficacy of Chinese materia medica intervention on Parkinson’s disease with sleep disorder.Methods:Some Chinese and English databases were comprehensively searched for studies on randomized clinical trials of treating Parkinson’s disease with sleep disorders in traditional Chinese medicine.The retrieval time was from the establishment of the databases to August 2021,and RevMan 5.4.1 software was used for data analysis.Results:23 studies were included,with a sample size of 1483 cases.The meta-analysis showed that there were statistically significant differences between pure Western medicine and Chinese materia medica in clinical efficacy(P<0.00001),TCM syndrome score(P<0.00001),PSQI score(P<0.00001),UPDRSⅢ(P<0.00001),PDSS score(P<0.00001),ESS score(P=0.02),and PDQ-39 score(P<0.00001).Conclusion:Chinese materia medica intervention on Parkinson’s disease with sleep disorder can better ameliorate sleep disturbance of patients than Western medicine only,and there are fewer reports of adverse reactions.
