摘要
目的探讨开展中药饮片处方点评对促进临床合理用药以及提高药学服务质量的作用。方法对该院1000张中药饮片处方进行回顾性分析,其中2020年1—6月500张处方未进行处方点评(对照组),2020年7—12月500张处方则开展处方点评(研究组),比较实施处方点评前后适应证不适宜、用法用量不适宜、重复用药、大处方、用药禁忌、配伍禁忌的发生率,并统计实施前后患者的不良反应发生情况对比,另外调查患者对诊疗和取药过程的满意度。结果研究组实施处方点评后适应证不适宜、用法用量不适宜、重复用药、大处方、配伍禁忌发生率分别为0.40%、0.20%、0.40%、0.20%、0.00%,均低于实施前对照组的1.80%、2.00%、2.20%、1.60%、1.40%,差异有统计学意义(χ^(2)=4.504、7.446、6.313、4.036、5.179,P<0.05)。研究组实施后不良反应发生率为1.00%,低于实施前对照组的4.00%,差异有统计学意义(χ^(2)=9.231,P<0.05)。研究组实施后患者对诊疗和取药过程满意度为99.20%,高于实施前对照组的94.20%,差异有统计学意义(χ^(2)=19.586,P<0.05)。结论开展中药饮片处方点评工作能够促进临床合理用药,可降低患者不良反应风险,有利于提高患者的满意度。
Objective To explore the effect of developing Chinese herbal decoction pieces prescription reviews on promoting clinical rational drug use and improving the quality of pharmaceutical services.Methods A retrospective analysis was performed on 1000 prescriptions of Chinese herbal decoction pieces in the hospital,of which 500 prescriptions from January to June 2020 did not receive prescription reviews(control group),and 500 prescriptions from July to December 2020 were provided with prescription reviews(study group),compared the incidence of unsuitable indications,unsuitable usage and dosage,repeated medication,large prescriptions,medication contraindications,and incompatibility before and after the implementation of prescription reviews,and the incidence of adverse reactions of patients before and after the implementation was compared,and the satisfaction of patients with the process of diagnosis and treatment and drug collection was also investigated.Results The incidence rates of inappropriate indications,inappropriate usage and dosage,repeated medication,large prescriptions,and incompatibility in the study group were 0.40%,0.20%,0.40%,0.20%,0.00%,respectively,which were lower than 1.80%,2.00%,2.20%,1.60%,1.40%of the control group before implementation,and the differences were statistically significant(χ^(2)=4.504,7.446,6.313,4.036,5.179,P<0.05).The incidence of adverse reactions in the study group after implementation was 1.00%,which was lower than 4.00%in the control group before implementation,and the differences were statistically significant(χ^(2)=9.231,P<0.05).After the implementation,the patients in the study group were 99.20%satisfied with the process of diagnosis,treatment and drug collection,which was higher than 94.20%in the control group before implementation,and the differences were statistically significant(χ^(2)=19.586,P<0.05).Conclusion Carrying out the evaluation of the prescription of Chinese herbal decoction pieces can promote clinical rational drug use,reduce the risk of adverse reactions of patients,and improve the satisfaction of patients.
