摘要
目的观察加味麻杏石甘汤治疗社区获得性肺炎的临床疗效。方法收集2019年1月—2020年8月诊断为社区获得性肺炎痰热壅肺证的102例患者,随机分为对照组与中药组各51例,对照组予规范西医治疗。中药组在对照组治疗基础上加用加味麻杏石甘汤。比较2组治疗前后炎症指标、心功能、肝功能、肾功能、疗效证候变化及主要症状体征改善时间。结果治疗前,2组患者白细胞、中性粒细胞百分比、C反应蛋白、血清降钙素原差异无统计学意义(P>0.05)。治疗后,中药组中医疗效评定显著优于对照组(P<0.05)。与对照组比较,中药组中性粒细胞百分比、C反应蛋白、血清降钙素原显著下降(P<0.05),患者退热时间、咳嗽、咳痰缓解时间显著缩短(P<0.05),谷草转氨酶、谷丙转氨酶、肌酐、肌酸激酶、肌酸激酶同工酶水平均较低(P<0.05)。结论加味麻杏石甘汤可以明显降低患者血清炎性反应指标,较快改善症状,起到保护肝、肾、心脏功能作用。
Objective To evaluate the clinical efficacy of modified Maxing Shigan decoction in the treatment of community acquired pneumonia.Methods 102 patients with community acquired pneumonia of syndrome of phlegm-heat obstructing lung from January 2019 to August 2020 were divided into the control group and the Chinese medicine group.The control group was given standard treatment according to the 2016 guidelines for adult community-acquired pneumonia.On the basis of treatment,51 patients in the Chinese medicine group were treated with modified Maxing Shigan decoction,1 dose per day,for 5 consecutive days.The changes of white blood cell count,neutrophil percentage,C-reactive protein and serum procalcitonin level before and after treatment were detected,and the changes of curative symptoms and signs and the improvement time of main symptoms and signs were observed in the two groups.At the same time,aspartate aminotransferase,alanine transaminase,serum creatinine,creatine kinase levels in the two groups were compared.Results Before treatment,there were no significant differences in the percentages of white blood cells,neutrophils,C-reactive protein and serum procalcitonin between 2 groups(P>0.05).After treatment,the evaluation of traditional Chinese medicine efficacy in Chinese medicine group was significantly better than that in control group(P<0.05).Compared with the control group,the percentage of neutrophil,C-reactive protein and serum procalcitonin in the Chinese medicine group were significantly decreased(P<0.05),the fever relieving time,cough and expectoration relieving time were significantly shortened(P<0.05),and the levels of aspartate aminotransferase,alanine aminotransferase,creatine kinase and creatine kinase isoenzyme were lower in the Chinese medicine group(P<0.05).Conclusion Modified Maxing Shigan decoction can significantly reduce the serum inflammatory response indicators of patients,improve symptoms quickly,and play a role in protecting the liver,kidney and heart functions.
