摘要
济川煎载于《景岳全书》,为明代医家张景岳首创,同时也是百首经典名方之一。经查阅相关文献发现,近年有关济川煎相关报道多集中于实验研究与临床试验,少有从文献角度对其进行分析。基于济川煎研究现状,本文采用文献计量学与文献研究的方法对济川煎原文、剂量古今折算、功效、古今临床应用等方面进行考辨分析。结果发现,济川煎除原文所载治疗虚秘、产后便秘、气瘕等外,清代医家还将其应用于小儿便秘、目病、燥结、胸中痞满(阴结于下)、冬温兼伏暑伤阴等症,现代研究发现其主治病症主要为“便秘”“眩晕”“中风”“腹痛”“消渴”“痞满”“郁证”“虚劳”等。该方的功效可标记为温肾益精,润肠通便。在尊重原方用量、保证处方安全、方便临床应用及标准化生产的前提下,建议该方临床参考剂量为当归12 g,牛膝6 g,肉苁蓉9 g,泽泻6 g,升麻3 g,枳壳3 g。该研究立足于古今文献考证,对明确济川煎临床应用,阐释其临床功效具有重要意义,以期为济川煎的二次开发及其他经典名方的考证提供思路。
Jichuan Decoction, which was recorded in Jing Yue Quan Shu, was first created by ZHANG Jingyue, famous doctor in Ming Dynasty, and it is also one of the classic prescriptions. After consulting the relevant literature, the author found that in recent years, the relevant reports on Jichuan Decoction mostly focused on experimental research and clinical trials, and little analysis from textual research. Based on research status of Jichuan Decoction, the author uses bibliometrics method to analyze the original text, dose conversion, efficacy and clinical application of Jichuan Decoction. The results showed that in addition to the treatment of deficient constipation, postpartum constipation and Qi abdominal mass recorded in the original lines, Jichuan Decoction was also used to treat pediatric constipation, eye disease, accumulation of dry feces, fullness in the chest(Yin accumulating below), as well as winter warmth and lurking summer-heat damaging Yin by doctors in Qing Dynasty. Modern studies have found that its main indications are constipation, dizziness, stroke, abdominal pain, Xiaoke, fullness, depression and comsumptive diseases. The efficacy of the prescription can be marked as warming Kidney and benefiting essence, moistening intestines and moving stools. On the premise of abiding the dosage of the original prescription, ensuring the safety of the prescription and facilitating clinical application and production, the author suggests that the clinical dosage of the prescription can be Dang Gui(12 g), Niu Xi(6 g), Rou Cong Rong(9 g), Ze Xie(6 g), Sheng Ma(3 g) and Zhi Qiao(3 g). Based on the textual research of ancient and modern literature, this study is of great significance to clarify the clinical application of Jichuan Decoction and prove its clinical efficacy, in order to provide ideas for the secondary development of Jichuan Decoction and the textual research of other classic prescriptions.
