摘要
目的分析超声造影引导肺活检穿刺术快速现场细胞学评价在肺外周病变诊断中的应用价值。方法100例存在肺外周病变的住院患者,均行超声造影引导下肺穿刺活检术,其中42例在穿刺后进行快速现场细胞学评价(ROSE组)、58例患者未进行快速现场细胞学评价(非ROSE组)。以临床最终诊断为肺外周病变诊断的金标准,绘制受试者工作特征曲线(ROC)测算超声造影引导穿刺行快速现场细胞学评价方法诊断肺外周病变的敏感度、特异度、假阳性率、阳性预测值及阴性预测值等。结果ROSE组最终诊断为恶性肿瘤31例、良性病变11例。ROSE组初步诊断结果为恶性肿瘤30例、良性病变12例,ROSE组穿刺诊断肺外周病变的阳性率为97.62%(41/42)。非ROSE组最终诊断为恶性肿瘤35例、良性病变23例,非ROSE组初步诊断结果为恶性肿瘤25例、良性病变33例,非ROSE组穿刺诊断肺外周病变的阳性率为86.21%(50/58)。与非ROSE组比较,ROSE组穿刺诊断肺外周病变的阳性率高(P<0.05)。快速现场细胞学评价在超声造影下肺穿刺活检术后诊断肺外周病变的灵敏度为96.77%,特异度为100%,假阳性率为3.23%,阳性预测值为100.00%,阴性预测值为91.67%。与非ROSE组比较,ROSE组穿刺次数少(P<0.05)。结论超声造影引导下肺穿刺活检术后快速现场细胞学评价诊断肺外周病变的灵敏度、特异度均较高,且明确诊断所需穿刺次数少。
Objective To investigate the application value of contrast-enhanced ultrasound(CEUS)combined with cytological rapid on-site evaluation(C-ROSE)in the diagnosis of peripheral pulmonary focal lesions.Methods Totally 100 patients with peripheral pulmonary lesions underwent contrast-enhanced ultrasound-guided lung biopsy.C-ROSE was performed in 42 patients after puncture(ROSE group),and 58 patients(non-ROSE group)did not receive C-ROSE.The final diagnostic examination was the gold standard for the diagnosis of peripheral pulmonary diseases.Receiver operating characteristic(ROC)curve was drawn to measure the sensitivity,specificity,false positive rate,positive predictive value and negative predictive value of C-ROSE for the diagnosis of pulmonary peripheral lesions after CEUS-guided puncture.Results In the ROSE group,31 cases were diagnosed as having malignant tumors and 11 cases as benign lesions.The primary diagnosis of the ROSE group showed that malignant tumor was found in 30 cases and benign lesion in 12 cases.The positive rate of the ROSE group was 97.62%(41/42).In the non-ROSE group,35 patients were diagnosed as hav⁃ing malignant tumors and 23 as benign lesions.The primary diagnosis results showed that the malignant tumor was found in 25 cases and benign lesion in 33 cases.The positive rate of the non-ROSE group was 86.21%(50/58).Compared with the non-ROSE group,ROSE group had higher positive rate of puncture diagnosis of pulmonary peripheral lesions(P<0.05).The sensitivity,specificity,false positive rate,positive predictive value,and negative predictive value of C-ROSE in the diagnosis of peripheral pulmonary lesions after lung biopsy under CEUS were 96.77%,100%,3.23%,100.00%and 91.67%,respectively.Compared with the non-ROSE group,ROSE group had fewer puncture times(P<0.05).Con⁃clusion C-ROSE for the diagnosis of pulmonary peripheral lesions after lung biopsy guided by CEUS has higher sensitivi⁃ty and specificity,and has less puncture times for definite diagnosis.
