摘要
目的探讨手法配合补肾壮腰方治疗退行性腰椎管狭窄症的疗效及对患者腰痛程度和腰椎功能的影响。方法选取2019年3月至2021年1月河源市中医院收治的80例老年退行性腰椎管狭窄症患者为研究对象,根据随机数表法分为观察组和对照组,每组40例。对照组患者采用单纯手法治疗,观察组患者给予手法联合补肾壮腰方治疗,疗程均为9周。比较两组患者的治疗效果,治疗前后的中医症状评分、腰痛改善情况[腰痛奥斯威斯指数(ODI)评分]、腰椎功能[日本骨科学会(JOA)评分]及步行距离。结果观察组患者的治疗总有效率为95.0%,明显高于对照组的80.0%,差异有统计学意义(P<0.05);治疗后,观察组患者的中医症状评分为(8.26±1.86)分,明显低于对照组的(18.67±3.29)分,ODI为(22.52±2.72)分,明显低于对照组的(34.23±3.86)分,差异均有统计学意义(P<0.05);治疗后,观察组患者的JOA评分为(23.77±4.73)分,明显高于对照组的(17.60±3.51)分,步行距离为(550.56±28.59)m,明显长于对照组的(410.67±27.76)m,差异均有统计学意义(P<0.05)。结论手法配合补肾壮腰方治疗退行性腰椎狭窄能有效提高患者的治疗效果,改善中医症状,降低患者腰痛程度。
Objective To observe the therapeutic effect of manipulation combined with Bushen Zhuangyao Fang on degenerative lumbar spinal stenosis,and its effect on the degree of low back pain and lumbar function.Methods Eighty elderly patients with degenerative lumbar spinal stenosis treated in Heyuan Hospital of Traditional Chinese Medicine from March 2019 to January 2021 were selected as the research objects.According to the random number table method,they were divided into two groups,with 40 patients in each group.The control group was treated with manipulation alone,and the experimental group was treated with manipulation combined with Bushen Zhuangyao Fang.The course of treatment lasted 9 weeks.After treatment,the total effective rate were compared,the TCM symptom improvement,pain improvement(ODI),lumbar spine ability(JOA),and walking distance of the two groups before and after treatment were compared.Results After treatment,the total effective rate of the experimental group was 95.0%,significantly higher than 80.0%in the control group(P<0.05).After treatment,TCM symptom score and ODI score in the experimental group were(8.26±1.86)points and(22.52±2.72)points,significantly lower than(18.67±3.29)points and(34.23±3.86)points in the control group(P<0.05).After treatment,the JOA score was(23.77±4.73)points in the experimental group,significantly higher than(17.60±3.51)points in the control group,and the walking distance was(550.56±28.59)m,significantly longer than(410.67±27.76)m in the control group(P<0.05).Conclusion The application of manipulation combined with Bushen Zhuangyao Fang in patients with degenerative lumbar stenosis can effectively improve the therapeutic effect,improve the symptoms of traditional Chinese medicine,and reduce the degree of pain.
