摘要
目的观察五音音乐振动理疗仪辅助治疗小儿痉挛型脑瘫的临床疗效。方法选取2019年11月至2020年3月在广州中医药大学附属南海妇产儿童医院儿童康复科门诊及住院部治疗的痉挛型脑瘫患儿80例,按随机数字表法分为对照组和治疗组,各40例。对照组予常规康复治疗,治疗组在对照组治疗方法的基础上联合五音音乐振动理疗仪进行治疗,2组均治疗60 d后统计疗效。结果治疗前2组双下肢肌张力评分比较,差异均无统计学意义(P>0.05),具有可比性;治疗后,2组双侧肌张力评分均较同组治疗前明显降低,差异有统计学意义(P<0.05),且治疗组双侧肌张力评分降低更显著(P<0.05)。治疗前2组双下肢综合痉挛量表(CSS)评分比较,差异无统计学意义(P>0.05),具有可比性;治疗后2组双下肢CSS评分均较同组治疗前明显降低,差异有统计学意义(P<0.05),且治疗组降低更显著(P<0.05)。治疗前2组双下肢关节活动度比较,差异无统计学意义(P>0.05),具有可比性;治疗后对照组双下肢关节活动度改善不明显,与同组治疗前比较,差异无统计学意义(P>0.05);治疗组双侧足背屈快角和足背屈慢角均明显减小,双侧髋外展角明显增大,与同组治疗前及对照组治疗后比较,差异均有统计学意义(P<0.05)。结论五音音乐振动理疗仪辅助治疗小儿痉挛型脑瘫,可明显缓解肌肉痉挛,降低肌张力,改善关节活动度,并且患儿依从性高,值得临床推广应用。
Objective To observe the clinical efficacy of five-tone music vibration physiotherapeutic instrument in adjuvant treatment of children with spastic cerebral palsy.Methods From December 2019 to March 2020,80 cases of children with spastic cerebral palsy treated in the outpatient service and inpatient department of children’s rehabilitation in Nanhai Obstetrics,Gynecology and Children’s Hospital Affiliated to Guangzhou University of Chinese Medicine were divided into control group and treatment group according to random number table method,with 40 cases in each group.The control group was given routine rehabilitation therapy,on this basis,the treatment group was combined with five-tone music vibration physiotherapeutic instrument for treatment.After both groups received treatment for 60 days,we kept statistics about the efficacy.Results Before treatment,there was no statistically significant difference in muscular tension of lower limbs score between the 2 groups(P>0.05),indicating comparability.After treatment,bilateral muscular tension score in both groups was significantly lower than that in the same group before treatment,and the difference was statistically significant(P<0.05),moreover,bilateral muscular tension score in the treatment group decreased more significantly(P<0.05).Before treatment,there was no statistically significant difference in the scores of bilateral lower limb comprehensive spasm scale(CSS)between the 2 groups(P>0.05),indicating comparability.After treatment,CSS scores of bilateral lower limbs in both groups were significantly lower than those in the same group before treatment,and the difference was statistically significant(P<0.05),moreover,the decrease was more significant in the treatment group(P<0.05).Before treatment,there was no statistically significant difference in the range of motion(ROM)of bilateral lower limbs between the 2 groups(P>0.05),indicating comparability.After treatment,the ROM of bilateral lower limbs in the control group was not significantly improved,and there was no statistically significant difference than that in the same group before treatment(P>0.05).In the treatment group,the bilateral dorsiflexion fast angle of foot and dorsiflexion slow angle of foot decreased significantly,while the bilateral hip abduction angle increased significantly,and the differences were statistically significant than those in the same group before treatment and those in the control group after treatment(P<0.05).Conclusion It can markedly relieve muscle spasm,reduce muscular tension,improve ROM of joints to use five-tone music vibration physiotherapeutic instrument in adjuvant treatment of children with spastic cerebral palsy,with high compliance of children patients,which is worthy of clinical promotion and application.
