摘要
目的:探讨老年股骨颈骨折患者全髋关节置换术后下肢不等长的影响因素。方法:选取120例行全髋关节置换术的老年股骨颈骨折患者为研究对象。术后随访3个月,统计患者下肢不等长发生情况,并对导致老年股骨颈骨折患者全髋关节置换术后发生下肢不等长的影响因素进行单因素和多因素Logistic回归分析。结果:术后随访3个月,120例老年股骨颈骨折患者术后发生下肢不等长25例,发生率为20.83%;单因素分析结果显示,下肢不等长患者与下肢等长患者的年龄、麻醉方式、合并糖尿病情况比较,差异均有统计学意义(P<0.05);下肢不等长患者与下肢等长患者的性别、关节炎、置换髋关节类型、术后主动锻炼、术中使用肌松剂、合并高血压和贫血情况比较,差异均无统计学意义(P>0.05);Logistic回归分析结果显示,年龄>80岁、椎管内麻醉、合并糖尿病均为影响老年股骨颈骨折患者全髋关节置换术后发生下肢不等长的独立危险因素(OR>1,P<0.05)。结论:年龄>80岁、椎管内麻醉、合并糖尿病为影响老年股骨颈骨折患者全髋关节置换术后发生下肢不等长的独立危险因素,临床可采取针对性措施以避免术后下肢不等长的发生。
Objective:To explore influencing factors of leg length discrepancy after total hip arthroplasty in elderly patients with femoral neck fractures.Methods:120 elderly patients with femoral neck fractures undergone total hip arthroplasty were selected as the research objects,and were followed up for 3 months.The occurrence of leg length discrepancy was statistically analyzed.Univariate analysis and multivariate Logistic regression were used to analyze the influencing factors of leg length discrepancy after total hip arthroplasty in these elderly patients with femoral neck fractures.Results:After 3 months of follow-up,among the 120 elderly patients with femoral neck fractures,the leg length discrepancy occurred in 25 cases with an incidence of 20.83%.The results of univariate analysis showed:there were statistically significant differences in the age,anesthesia mode,and combined diabetes among the patients with leg length discrepancy and those with equal leg length(P<0.05);and there were no statistical differences in the gender,arthritis,type of hip replacement,postoperative active exercise,intraoperative use of muscle relaxants,hypertension and anemia among the patients with leg length discrepancy and those with equal leg length(P>0.05).The multivariate logistic regression analysis showed that age>80 years,intraspinal anesthesia,and combined diabetes were all independent risk factors for the leg length discrepancy after total hip arthroplasty in the elderly patients with femoral neck fractures(OR>1,P<0.05).Conclusions:Age>80 years,intraspinal anesthesia,and combined diabetes are all independent risk factors for the leg length discrepancy after total hip arthroplasty in the elderly patients with femoral neck fractures.Therefore,clinical measures should be taken to avoid the occurrence of the leg length discrepancy after the surgery.
