摘要
目的比较“土”字法和三段法在下颌水平阻生智齿的微创拔除术中的临床效果。方法选择2019年6月至2020年12月我科门诊的100例患者的200颗左右基本对称的下颌水平阻生智齿作为研究对象,采用自身左右对照原则将每例患者的下颌阻生智齿随机分为“土”字法组和三段法组,对手术时间、去骨量、术后疼痛程度及持续时间、术后肿胀程度及持续时间、术后张口受限程度及持续时间、干槽症发生率、患者主观满意度进行比较观察和统计学分析。结果在下颌水平阻生智齿的微创拔除中,“土”字法组的手术时间[(13.580±2.847)min]与三段法组[(13.650±2.676)min]相近,差异无统计学意义(P>0.05);“土”字法组的去骨量为(22.75±4.36)mm^(2),少于三段法组的(27.30±4.97)mm^(2),差异有统计学意义(P<0.05);“土”字法组的术后疼痛为(12.26±2.38)分,低于三段法组的(16.00±2.56)分,差异有统计学意义(P<0.05);“土”字法组的术后肿胀为(15.88±3.26)分,低于三段法组的(19.46±4.94)分,差异有统计学意义(P<0.05);“土”字法组的术后张口受限为(30.97±4.41)分,低于三段法组的(36.51±4.99)分,差异有统计学意义(P<0.05);“土”字法组的干槽症发生率(5.0%)与三段法(6.0%)相近,差异无统计学意义(P>0.05)“土”字法组的患者主观满意度高于三段法组,差异有统计学意义(P<0.05)。结论对于下颌水平阻生的阻生智齿的微创拔除,“土”字法较三段法术后反应更轻微,持续时间更短,患者主观感受更舒适,也更容易被患者接受。
Objective To compare the clinical effects of"Tu"method and three-segment method in minimally invasive extraction of mandibular horizontal impacted wisdom teeth.Methods About 200 symmetrical mandibular horizontal impacted wisdom teeth from 100 patients in our clinic from June 2019 to December 2020 were selected as the research objects.According to the principle of self-control,the mandibular impacted wisdom teeth of each patient were randomly divided into"Tu"method group and three-segment method group.The time of operation,the amount of bone removed,the degree and duration of postoperative pain,the degree and duration of postoperative swelling,the degree and duration of postoperative mouth opening,the incidence of dry socket and the subjective satisfaction of patients were compared and statistically analyzed.Results In the minimally invasive extraction of horizontally impacted wisdom teeth of the mandible,the operation time of"Tu"method group([13.580±2.847]min)was similar to that of three-segment method group([13.650±2.676])min,with no statistically significant difference(P>0.05).The bone removal amount of"Tu"group([22.75±4.36]mm^(2))was less than that of three-segment group([27.30±4.97]mm^(2)),and the difference was statistically significant(P<0.05).The postoperative pain in the"Tu"group([12.26±2.38]points)was lower than that in the three-segment group([16.00±2.56]points),and the difference was statistically significant(P<0.05).The postoperative swelling in the"Tu"group([15.88±3.26]points)was lower than that in the three-segment group([19.46±4.94]points),and the difference was statistically significant(P<0.05).The limitation of mouth opening after operation in the"Tu"group([30.97±4.41]points)was lower than that in the three-segment group([36.51±4.99]points),and the difference was statistically significant(P<0.05).The incidence of dry socket in the"Tu"method group(5.0%)was similar to that in the three-segment group(6.0%),but the difference was not statistically significant(P>0.05).The subjective satisfaction of patients in the"Tu"group was higher than that in the three-segment group,and the difference was statistically significant(P<0.05).Conclusion For minimally invasive extraction of impacted wisdom teeth with horizontal mandibular impaction,patients after"Tu"method have milder response than those after three-segment method,the duration is shorter,the subjective feeling of patients is more comfortable,and it is easier to be accepted by patients.
