人体成分和血生化指标对鼻咽癌患者放疗期间营养状况预测的探讨 被引量:3

An Investigation of Body Composition and Biochemical Indexes in Prediction of Malnutrition in Patients with Nasopharyngeal Carcinoma

摘要 目的研究鼻咽癌病人放疗过程中营养情况与人体指标及血生化参数关联性,分析评估营养不良的最佳指标。方法随机调查2019年—2020年苏州大学附属第一医院收治的31例鼻咽癌病人放疗过程中的PG-SGA评分、人体成分变化,血生化指标变化;通过Logistic估测影响该类病人营养情况的关键因素。结果放疗过程中主要指标包括:重量、BMI、脂肪含量均随时间增加出现不同速度的降低,血生化指标中白细胞、血红蛋白、总蛋白、白蛋白等含量均出现减少,患者PGSGA评分逐渐升高(P<0.05)。因素分析显示体重丢失、血白蛋白以及前白蛋白水平对患者营养情况起到了关键作用(P<0.05)。结论该类病人在放化疗过程中的营养情况显著恶化,构建营养估测模型能够综合且精准评判病人的营养情况。 Objective To study the correlation between nutritional status and human body indexes and blood biochemical parameters in patients with nasopharyngeal carcinoma during radiotherapy,and to analyze the best index to evaluate malnutrition.Methods The changes of PG-SGA score,body composition and blood biochemical indexes of 31 patients with nasopharyngeal carcinoma treated in the First Affiliated Hospital of Soochow University from 2019 to 2020 were randomly investigated.The key factors affecting the nutritional status of these patients were estimated through Logistic analysis.Results The main indexes during radiotherapy including weight,BMI and fat content decreased at different rates with the increase of time,the contents of leukocyte,hemoglobin,total protein and albumin in blood biochemical indexes decreased,and the PG-SGA score increased gradually(P<0.05).Factor analysis showed that weight loss,serum albumin and prealbumin levels played a key role in patients'nutritional status(P<0.05).Conclusion The nutritional status of these patients deteriorated significantly during radiotherapy and chemotherapy.The construction of nutritional estimation model can comprehensively and accurately evaluate the nutritional status of patients.
