摘要
基于伏邪理论,阐述川崎病的发病机制,认为小儿正气不足、邪气内伏是其内因,而外邪侵袭是其诱因,常因内外合邪而发病,且有明显的季节性特点。治疗上应以温病理论为指导,分期论治。初期气分热盛,治宜重用凉透,予柴葛解肌汤加减;中期邪热耗血动血,治宜凉散,予清瘟败毒饮加减;后期温毒久羁,耗伤气阴,治宜补肾益精、补气活血,予犀角地黄汤加减。此外,结合该病易损伤心脉的特点,应重视活血药的应用,既能提高疗效,又能预防病情加重。
Based on the theory of latent pathogenic factors theory,this paper expounds the pathogenesis of Kawasaki disease(KD).It holds that the deficiency of healthy qi and the internal latency of pathogenic factors in children are the internal causes thereof,while the invasion of exogenous pathogenic factors is the inducement thereof,which often comes on due to the combination of internal and external pathogenic factors,and has obvious seasonal characteristics.The treatment should be guided by the theory of febrile disease and treated by stages.At the initial stage of excessive qi at qifen,it is advised that cooling and letting out pathogenic factors method be heavily used for treatment and modified Chaige Jieji Tang(柴葛解肌汤)be given;In the middle stage of pathogenic heat consuming blood and stirring blood,it is advised that cooling and dispelling method be used for treatment and modified Qingwen Baidu Yin(清瘟败毒饮)be given;At the later stage of prolonged lingering warm-toxin causing consumption of qi and yin,it is advised that reinforcing kidney and replenishing essence as well as invigorating qi to activate blood method be used for treatment and modified Xijiao Dihuang Tang(犀角地黄汤)be given.In addition,combined with the characteristics that the disease is prone to impair the heart vessels,we should attach importance to the application of activating blood medicinals,which can not only improve the curative effect,but also prevent the aggravation of the disease.
