摘要
目的探讨孟鲁司特钠联合阿奇霉素治疗小儿咳嗽变异性哮喘(CVA)的效果及安全性。方法将2018年3月至2020年3月我院收治的90例CVA患儿按随机数表法分为对照组(50例)与研究组(40例)。对照组给予阿奇霉素治疗,研究组给予孟鲁司特钠联合阿奇霉素治疗。比较两组的肺功能指标、炎症因子指标及并发症发生情况。结果治疗后,两组25%肺活量时最大呼气流速(MEF25)、50%肺活量时最大呼气流速(MEF50)、最大中段呼气流速(MMEF25~75)、第1秒用力呼气容积(FEV1)/用力肺活量(FVC)均升高,且研究组高于对照组,差异具有统计学意义(P<0.05)。治疗后,两组肿瘤坏死因子-α(TNF-α)、C反应蛋白(CRP)水平及白细胞计数(WBC)、中性粒细胞百分比(NEUT%)均降低,且研究组低于对照组,差异具有统计学意义(P<0.05)。研究组治疗期间的不良反应总发生率低于对照组,差异具有统计学意义(P<0.05)。结论孟鲁司特钠联合阿奇霉素治疗CVA患儿可有效改善其肺功能,减轻气道炎症反应,且具有较高的安全性。
Objective To study the effect and safety of montelukast sodium combined with azithromycin in the treatment of cough variant asthma(CVA)in children.Methods Ninety children with CVA treated in our hospital from March 2018 to March 2020 were divided into control group(50 cases)and study group(40 cases)according to the random number table method.The control group was treated with azithromycin,and the study group was treated with montelukast sodium combined with azithromycin.The pulmonary function indexes,inflammation factor indexes and complications were compared between the two groups.Results After treatment,maximum expiratory flow at 25%vital capacity(MEF25),maximum expiratory flow at 50%vital capacity(MEF50),maximum middle expiratory flow(MMEF25-75),forced expiratory volume in the first second(FEV1)/forced vital capacity(FVC)increased in both groups,those in the study group were higher than the control group,and the differences were statistically significant(P<0.05).After treatment,tumor necrosis factor-α(TNF-α),C-reactive protein(CRP)levels,white blood cell count(WBC)and percentage of neutrophils(NEUT%)decreased in both groups,those in the study group were lower than the control group,and the differences were statistically significant(P<0.05).The total incidence of adverse reactions during treatment in the study group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Montelukast sodium combined with azithromycin in the treatment of children with CVA can effectively improve pulmonary function and reduce airway inflammation response,with higher safety.
