摘要
目的总结良性和恶性原发性卵巢肿瘤临床案例的超声特点,分析彩色多普勒超声技术对于良恶性肿瘤的临床辨别价值,为良恶性卵巢肿瘤的早期无创鉴别诊断提供参考依据。方法以2020年4月~2021年8月收治的116例卵巢肿瘤患者为研究对象,对患者术前彩色多普勒超声检查和术后病理组织学检查结果进行回顾性对比分析,总结不同性质和类型的卵巢良恶性肿瘤的超声影像规律,进一步评价彩色多普勒超声诊断卵巢恶性肿瘤的灵敏度及特异性等诊断效果。结果116例患者中患良性卵巢肿瘤者110例,占比94.83%,其余6例为恶性卵巢肿瘤患者,占比5.17%,不同卵巢肿瘤的超声图谱具有不同声像特点,其中良性卵巢肿瘤的子宫内膜异位囊肿、成熟性囊性畸胎瘤的诊出率显著高于其它类型(P<0.05)。彩色多普勒超声诊断恶性卵巢肿瘤的灵敏度为50%,特异性为98.18%。阳性预测值为60%,阴性预测值为97.30%。结论分析良恶性卵巢肿瘤的声像图特点和差别,有助于提高对卵巢肿瘤性质和类型的诊断准确率,提示彩色多普勒超声技术在卵巢肿瘤良恶性辨别方面有重要参考价值。
Objective To summarize the ultrasonic characteristics of clinical cases of benign and malignant primary ovarian tumors by color Doppler ultrasound,and analyze their clinical differential values,and further provide reference basis for early noninvasive diagnosis of benign and malignant ovarian tumors.Methods 116 patients with ovarian tumors treated in our hospital from April 2020 to August 2021 were chosen as the research objects.Their results of preoperative color Doppler ultrasonography and postoperative histopathology were retrospectively compared and analyzed to know the ultrasonic imaging rules of different types of benign and malignant ovarian tumors,and the sensitivity and specificity of color Doppler ultrasound in the diagnosis of ovarian malignant tumors.Results Among the 116 patients,110 patients had benign ovarian tumors,accounting for 94.83%,and the other 6 patients were malignant ovarian tumors,accounting for 5.17%.The ultrasonic maps of different ovarian tumors had different ultrasonic characteristics.The diagnostic rates of Endometriosis cyst and Mature cystic teratoma of benign ovarian tumors were significantly higher than those of other types(P<0.05).The sensitivity and specificity of color Doppler ultrasound in the diagnosis of malignant ovarian tumors was 50%and 98.18%.The positive and negative predictive value were 60%,and 97.30%.Conclusion Analyzing the sonographic characteristics and differences between benign and malignant ovarian tumors is helpful to improve the diagnostic accuracy of the properties and types of ovarian tumors,it is suggesting that color Doppler ultrasound has important reference value in guiding the identification of benign and malignant ovarian tumors.
