摘要
目的探讨超声、CT、MRI联合血清糖类抗原125(CA125)对卵巢肿瘤性质的判断价值。方法回顾性分析295例卵巢肿瘤患者的完整的超声、CT、MRI、血清CA125检查资料和病理资料,观察卵巢良性和恶性肿瘤患者的超声、CT、MRI、血清CA125检查结果,以病理结果为金标准,比较以上各项单独检查及联合检查在卵巢良、恶性肿瘤检出率方面的差异。结果病理结果提示295例卵巢肿瘤患者中,良性192例、恶性103例。卵巢良、恶性肿瘤患者彩色多普勒超声图像的形态、包膜、回声、血流阻力RI值比较差异均有统计学意义(P<0.05)。卵巢良性肿瘤患者的CT检查病灶区最大标准摄取值(SUVmax)显著低于恶性肿瘤组患者,并且阳性率显著低于恶性肿瘤患者,差异均有统计学意义(P<0.05)。卵巢良、恶性肿瘤患者MRI图像在病灶境界、形态、分布、强化程度及成分方面差异均有统计学意义(P<0.05)。卵巢良性肿瘤患者的血清CA125水平显著低于恶性肿瘤组患者,并且阳性率显著低于恶性肿瘤患者,差异有统计学意义(P<0.05)。超声、CT、MRI及血清CA125联合检查对卵巢良、恶性肿瘤的检出率均显著高于以上各项单独检查,差异均有统计学意义(P<0.05)。结论卵巢良、恶性肿瘤的超声、CT、MRI、血清CA125均具有显著差异,超声、CT、MRI联合血清CA125对卵巢良、恶性肿瘤的检出率明显高于单独检查,其对肿瘤性质的判断准确性更高,可以为临床诊治工作提供可靠依据。
Objective To investigate the value of ultrasound,CT,MRI and Serum CA125 in judging the nature of ovarian tumors.Methods The clinical data of 295 patients with ovarian tumors diagnosed and treated were analyzed retrospectively,all patients had complete ultrasound,CT,MRI,serum CA125 examination data and pathological data,and observed the results of ultrasound,CT,MRI,and serum CA125 in patients with benign and malignant ovarian tumors,pathological results as the gold standard,the detection rates of benign and malignant ovarian tumors with the above individual examinations and combined examinations were compared.Results The pathological results showed that among the 295 patients with ovarian tumors,192 were benign and 103 were malignant.The RI values of color Doppler ultrasound images of patients with benign and malignant ovarian tumors were significantly different in morphology,capsule,echo and blood flow resistance(P<0.05).The maximum standard uptake value(SUVmax)of CT in patients with benign ovarian tumors was significantly lower than that of patients with malignant tumors,and the positive rate was significantly lower than that of patients with malignant tumors,the differences were statistically significant(P<0.05).There were statistically significant differences in MRI images of patients with benign and malignant ovarian tumors in the extent,shape,distribution,enhancement degree and composition of the lesion(P<0.05).The serum CA125 level of patients with benign ovarian tumors was significantly lower than that of patients with malignant tumors,and the positive rate was significantly lower than that of patients with malignant tumors,the differences were statistically significant(P<0.05).Ultrasound,CT,MRI and serum CA125 combined examinations had significantly higher detection rates of ovarian benign and malignant tumors than the individual inspection of above examinations,and the differences were statistically significant(P<0.05).Conclusion There are significant differences in ultrasound,CT,MRI and serum CA125 between benign and malignant ovarian tumors,the detection rate of ultrasound,CT,MRI combined with serum CA125 for ovarian benign and malignant tumors is significantly higher than that of individual examinations,and combined examinations have higher accuracy in judging the nature of tumors,it can provide a reliable basis for clinical diagnosis and treatment.
