中风康复胶囊治疗脑梗死颈动脉斑块形成的效果研究 被引量:2

摘要 目的探究脑梗死颈动脉斑块形成患者应用中风康复胶囊治疗的临床效果。方法80例脑梗死颈动脉斑块形成患者作为研究对象,按照抽签分组法分为对照组与观察组,每组40例。对照组患者采用常规方法进行西药治疗,观察组患者在对照组基础上加用中风康复胶囊治疗。比较两组患者治疗前后颈动脉斑块指标(颈动脉内膜中层厚度、斑块面积),治疗前后血脂指标[总胆固醇(TC)、甘油三脂(TG)、低密度脂蛋白胆固醇(LDL-C)],治疗前后血管内皮功能指标[血清一氧化氮(NO),血浆内皮素-1(ET-1)、血浆血管性血友病因子(vWF)、血清丙二醛(MDA)]。结果治疗后,观察组患者的颈动脉内膜中层厚度(0.59±0.06)mm、颈动脉斑块面积(0.57±0.04)cm^(2)小于治疗前的(1.76±0.08)mm、(3.85±0.08),对照组的(0.96±0.06)mm、(0.86±0.09)cm^(2)均小于治疗前的(1.78±0.14)mm、(3.87±0.06)cm^(2),且观察组颈动脉内膜中层厚度、颈动脉斑块面积均小于对照组,差异有统计学意义(P<0.05)。治疗后,观察组患者的TG、TC以及LDL-C分别为(1.17±0.23)、(3.08±0.45)、(1.42±0.37)mmol/L均低于本组治疗前的(1.95±0.42)、(6.46±0.83)、(3.89±0.46)mmol/L,对照组患者的TG、TC以及LDL-C分别为(1.63±0.35)、(4.26±0.63)、(1.97±0.43)mmol/L均低于本组治疗前的(2.07±0.36)、(6.56±0.78)、(3.93±0.48)mmol/L,且观察组TG、TC以及LDL-C均低于对照组,差异有统计学意义(P<0.05)。治疗后,观察组患者的NO为(401.91±20.53)μmol/L高于本组治疗前的(227.65±15.38)μmol/L,ET-1、vWF、MDA分别为(73.55±12.19)pg/L、(136.75±19.48)%、(4.54±0.49)nmol/L均低于本组治疗前的(120.28±15.80)pg/L、(233.62±35.24)%、(5.69±0.52)nmol/L,对照组患者的NO为(351.26±20.19)μmol/L高于本组治疗前的(228.19±16.05)μmol/L,ET-1、vWF、MDA分别为(86.36±14.17)pg/L、(191.57±35.26)%、(4.93±0.53)nmol/L均低于本组治疗前的(120.30±18.18)pg/L、(233.89±35.29)%、(5.79±0.69)nmol/L,观察组NO高于对照组,ET-1、vWF、MDA均低于对照组,差异有统计学意义(P<0.05)。结论脑梗死颈动脉斑块形成患者在常规治疗基础上增加中风康复胶囊,能够有效缩小脑梗死颈动脉斑块,同时降低患者血脂水平,有效改善高血脂对斑块形成的影响,改善患者血管内皮细胞功能,提高患者治疗效果。 Objective To investigate the clinical effect of Zhongfeng Kangfu capsule on carotid plaque in patients with cerebral infarction.Methods A total of 80 cases of cerebral infarction patients with carotid plaque were studied and divided into control group and observation group according to lottery method,with 40 cases in each group.Patients in the control group were treated with conventional western medicine,and patients in the observation group were treated with Zhongfeng Kangfu capsules on the basis of the control group.Both groups were compared in terms of carotid plaque indexes(carotid intima-media thickness,plaque area),lipid indexes[total cholesterol(TC),triglycerides(TG),low-density lipoprotein cholesterol(LDL-C)],vascular endothelial function indexes[serum nitric oxide(NO),plasma endothelin-1(ET-1),plasma von Willebrand factor(vWF),serum malondialdehyde(MDA)]before and after treatment.Results After treatment,the carotid artery intima-media thickness(0.59±0.06)mm and carotid artery plaque area(0.57±0.04)cm^(2) of the observation group were less than(1.76±0.08)mm and(3.85±0.08)of this group before treatment;the carotid artery intima-media thickness(0.96±0.06)mm and carotid artery plaque area(0.86±0.09)cm^(2) of the control group were less than(1.78±0.14)mm and(3.87±0.06)cm^(2) of this group before treatment;the carotid artery intima-media thickness and carotid artery plaque area in the observation group were less than those in the control group;all the differences were statistically significant(P<0.05).After treatment,TG,TC,and LDL-C of the observation group were(1.17±0.23),(3.08±0.45),(1.42±0.37)mmol/L,respectively,which were lower than(1.95±0.42),(6.46±0.83),(3.89±0.46)mmol/L of this group before treatment;TG,TC and LDL-C of the control group were(1.63±0.35),(4.26±0.63),(1.97±0.43)mmol/L,which were lower than(2.07±0.36),(6.56±0.78),(3.93±0.48)mmol/L of this group before treatment;TG,TC and LDL-C of the observation group were lower than those of the control group;all the differences were statistically significant(P<0.05).After treatment,NO(401.91±20.53)μmol/L of the observation group was higher than(227.65±15.38)μmol/L of this group before treatment,and ET-1(73.55±12.19)pg/L,vWF(136.75±19.48)%,and MDA(4.54±0.49)nmol/L were lower than(120.28±15.80)pg/L,(233.62±35.24)%and(5.69±0.52)nmol/L of this group before treatment;NO(351.26±20.19)μmol/L of the control group was higher than(228.19±16.05)μmol/L of this group before treatment,and ET-1(86.36±14.17)pg/L,vWF(191.57±35.26)%and MDA(4.93±0.53)nmol/L were lower than(120.30±18.18)pg/L,(233.89±35.29)%and(5.79±0.69)nmol/L of this group before treatment;NO of the observation group was higher than that of the control group,and ET-1,vWF,MDA were lower than those of the control group.All the differences were statistically significant(P<0.05).Conclusion For the treatment of carotid plaque in patients with cerebral infarction,the addition of Zhongfeng Kangkang capsules on the basis of conventional treatment can effectively reduce cerebral infarction carotid artery plaque,while reducing the patient’s blood lipid level,effectively improving the effect of high blood lipids on plaque formation,improving patients’vascular endothelial cell function,and improving patients’therapeutic effects.
