摘要
目的探讨错配修复蛋白MLH1、MSH2、MSH6、PMS2在结直肠癌组织中的表达及临床意义。方法收集2018年12月至2020年12月平煤神马医疗集团总医院病理科的62例结直肠癌标本。采用免疫组化染色法检测MLH1、MSH2、MSH6、PMS2蛋白表达情况。分析蛋白表达缺失与结直肠癌临床特征的关系,并进行生存曲线分析。结果 MLH1、MSH2、MSH6、PMS2蛋白表达缺失分别为4、3、3、8例,总缺失18例(29.03%)。错配修复蛋白表达缺失与性别、年龄、肿瘤部位、癌胚抗原表达、糖类抗原199表达无关联(P>0.05),与TNM分期、T分期、N分期、分化程度有明显关联(P<0.05)。MSH2与MSH6表达缺失具有相关性(c=0.545),MLH1与PMS2表达缺失具有相关性(c=0.437)。错配修复蛋白表达缺失的患者无进展生存期为9.6~27.0个月,蛋白表达阳性的患者无进展生存期为14.5~27.0个月,两组生存曲线比较差异有统计学意义(P<0.05)。结论错配修复蛋白MLH1、MSH2、MSH6、PMS2表达缺失与结直肠癌临床分期、分化程度、预后密切相关。
Objective To investigate the expression and clinical significance of mismatch repair proteins MLH1,MSH2,MSH6 and PMS2 in colorectal cancer tissues.Methods A total of 62 colorectal cancer specimens from Department of Pathology,General Hospital of Pingmei Shenma Medical Group from December 2018 to December 2020 were collected.The expressions of MLH1,MSH2,MSH6 and PMS2 were detected by immunohistochemical staining.The relationship between expression loss of proteins and clinical characteristics of colorectal cancer was analyzed by survival curve analysis.Results There were 4 cases,3 cases,3 cases and 8 cases with loss of MLH1,MSH2,MSH6 and PMS2,respectively.And there were 18 cases(29.03%)with expression loss.Expression loss of mismatch repair proteins was not related to gender,age,tumor site,expression of carcinoembryonic antigen and carbohydrate antigen 199(P>0.05);while it was significantly related to TNM staging,T staging,N staging and differentiation degree(P<0.05).Expression loss of MSH2 and MSH6 was correlated with each other(c=0.545);and expression loss of MLH1 and PMS2 was correlated with each other(c=0.437).The progression-free survival in patients with expression loss of mismatch repair proteins was 9.6-27.0 months,and it was 14.5-27.0 months in patients with positive expression.The differences in survival curves between the two groups were statistically significant(P<0.05).Conclusions Expression loss of mismatch repair proteins(MLH1,MSH2,MSH6,PMS2)is closely related to clinical staging,differentiation degree and prognosis of colorectal cancer.
