微柱凝胶试验免疫检测技术在输血科交叉配血中的应用价值 被引量:14

摘要 目的分析微柱凝胶试验(MGT)免疫检测技术在输血科交叉配血及血型鉴定中的应用效果。方法回顾性选取2019年3月至2021年2月平煤神马医疗集团总医院154例需输血患者,另取154例无偿献血者的血液标本。所有血液标本均采用MGT法、盐水试管法进行血型鉴定,并以MGT法、凝聚胺试验(MPT)检测交叉配血结果。记录不同方法检测结果,分析MGT法、盐水试管法鉴定结果,并比较MGT法、MPT法检测交叉配合成功率。结果盐水试管法、MGT法血型鉴定正反定型符合率及不符率比较,差异无统计学意义(P>0.05);MGT法检测交叉配血成功率(98.70%)高于MPT法检测(94.16%),交叉配血不合率(1.30%)低于MPT法检测(5.84%)(P<0.05);8例正反定型不符者中红细胞浓度异常2例(25.00%)、红细胞悬液不新鲜2例(25.00%)、血浆含纤维蛋白4例(50.00%),而2例MGT检测交叉配血不合者中自身抗体原因1例,不规则抗体原因1例。结论输血科交叉配血中采用MGT免疫检测技术进行检测具有较高敏感性、准确性,能降低交叉配血假阳性风险,且血型鉴定准确度良好,在临床血型鉴定、输血过程中有良好应用价值,可提高输血安全性。 Objective To analyze the application effect of microcolumn gel test(MGT)immunoassay technology in cross-matching blood transfusion and blood typing.Methods A retrospective selection of 154 patients requiring blood transfusion in General Hospital of Pingmei Shenma Group from March 2019 to February 2021 were retrospectively selected,and blood samples from 154 unpaid blood donors were taken.All blood samples were identified by the MGT method and the saline test tube method,and the results of cross matching were detected by the MGT method and manual polybrene test(MPT).The results of different methods were recorded and analyzed,the identification results of MGT method and saline test tube method were analyzed,and the success rate of cross-combination between MGT and MPT was compared.Results There was no statistical difference in the coincidence rate and non-conformance rate between the saline test tube method and the MGT method for blood typing identification(P>0.05).The success rate of MGT method was(98.70%)higher than that(94.16%)of MPT method,and the blood incompatibility rate(1.30%)was lower than that(5.84%)detected by the MPT method(P<0.05).Among the 8 cases of positive and negative stereotypes,there were 2 cases of abnormal red blood cell concentration(25.00%),2 cases red blood cell suspension was not fresh(25.00%),4 cases plasma contained fibrin(50.00%).Among the 2 cases of MGT crossmatch incompatibility,1 case was caused by autoantibodies and 1 case was caused by irregular antibodies.Conclusion The MGT immunoassay technique used in crossmatching blood in the department of blood transfusion has high sensitivity and accuracy,can reduce the risk of false positive crossmatching,and has good accuracy in blood group identification.It has good application value in clinical blood group identification and blood transfusion,can improve the safety of blood transfusion.
