摘要
目的探讨曲安奈德鼻喷雾剂联合鼻通丸治疗变应性鼻炎(AR)的疗效,并分析其对辅助性T细胞1(Th1)/辅助性T细胞2(Th2)和辅助性T细胞17(Th17)/调节性T细胞(Treg)细胞平衡的影响。方法选取2016年6月~2019年12月我院收治的AR患者136例,随机分为对照组和观察组,每组各68例。对照组予以曲安奈德鼻喷雾剂治疗,观察组在对照组基础上加用鼻通丸进行治疗,疗程14d。治疗后记录两组有效率和症状改善情况进行疗效评估。采用视觉模拟评分法(VAS)评分对两组治疗前后喷嚏、鼻塞、流涕、鼻痒各个症状进行评定,并计算其症状总评分。检测患者静脉血中白细胞介素2(IL-2)、γ-干扰素(IFN-γ)、白细胞介素4(IL-4)、白细胞介素10(IL-10)水平以及Th17、Treg细胞的百分比含量。结果观察组有效率(92.65%)高于对照组(76.47%)(P<0.05)。治疗后,两组喷嚏、鼻塞、流涕、鼻痒症状VAS评分和症状总评分均低于治疗前,且观察组均低于对照组(P<0.05)。治疗后,两组IL-2、IFY-γ、IL-10、Treg水平均高于治疗前,IL-4、Th17、Th17/Treg水平均低于治疗前,且观察组IL-2、IFY-γ、IL-10、Treg水平均高于对照组,IL-4、Th17、Th17/Treg水平均低于对照组(P<0.05)。结论曲安奈德鼻喷雾剂联合鼻通丸治疗AR疗效好,能够调节患者Th1/Th2和Th17/Treg细胞平衡,改善机体的免疫状态,减轻炎性反应,从而有效改善患者临床症状,值得推广。
Objective To investigate the efficacy of triamcinolone acetonide nasal spray combined with Bitong pill in the treatment of allergic rhinitis(AR),and to analyze the effect on the cell balance of helper T cell 1(Th1)/helper T cell 2(Th2)and T helper cell 17(Th17)/T regulatory cell(Treg).Methods A total of 136 patients with AR admitted to our hospital from June 2016 to December 2019 were selected and randomly divided into the control group(n=68)and the observation group(n=68).The control group was treated with triamcinolone nasal spray,and the observation group was treated with Bitong pills on the basis of the control group for 14 days.After treatment,record the effective rate and symptom improvement of the two groups for efficacy evaluation.The visual analog scale(VAS)score was used to evaluate the symptoms of sneezing,stuffy nose,runny nose,and itchy nose before and after treatment,and the total symptom score was calculated.Detect the levels of interleukin 2(IL-2),interferon gamma(IFN-γ),interleukin 4(IL-4),interleukin 10(IL-10)and Th17,Treg cells in the venous blood Percentage content.Results The effective rate of observation group(92.65%)was higher than that of control group(76.47%)(P<0.05).After treatment,the VAS scores and total symptoms of sneezing,nasal congestion,runny nose,and nasal itching symptoms in the two groups were lower than those before treatment,and the observation group were lower than the control group(P<0.05).After treatment,IL-2,Imit-γ,IL-10 and Treg levels in both groups were higher than before treatment,IL-4,Th17 and Th17/Treg levels were lower than before treatment,and IL-2,Imit-γ,IL-10 and Treg levels in the observation group were higher than those in the control group,and IL-4,Th17 and Th17/Treg levels were lower than those in the control group(P<0.05).Conclusion Triamcinolone acetonide spray combined with Bitong pill has good curative effect on AR,which can adjust the balance of Th1/Th2 and Th17/Treg cells of patients,improve the immune status of the body,reduce inflammatory reactions,and effectively improve the clinical symptoms of patients,which is worthy of promoting.
