摘要
目的:探究2型糖尿病(T2DM)患者血清分泌型卷曲蛋白5(sFRP5)水平与糖脂代谢、骨代谢及骨密度的关系。方法:选取T2DM患者150例(T2DM组),另选取同期OGTT试验证实为正常糖耐量(NGT)者108例(NGT组)。检测两组血清sFRP5水平,糖脂代谢指标[空腹血糖(FPG)、餐后2 h血糖(2hPG)、糖化血红蛋白(HbA1c)、空腹血清胰岛素(FINS)、胰岛素抵抗指数(HOMA-IR)、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、高度脂蛋白胆固醇(HDL-C)],骨代谢指标[N端骨钙素(N-MID)、Ⅰ型前胶原氨基端前肽(PINP)、Ⅰ型胶原羟基端肽β降解产物(β-CTX)]。双能X吸收法检测腰椎骨密度(BMD)。结果:T2DM组HbA1c、FPG、2hPG、FINS、HOMA-IR、TG、LDL-C、BMD均高于NGT组,HDL-C、sFRP5、N-MID、PINP、β-CTX水平均低于NGT组(P<0.05)。在剔除年龄影响后,T2DM患者血清sFRP5与HbA1c、FPG、2hPG、FINS、HOMA-IR、TG及BMD负相关(r=-0.439、-0.436、-0.448、-0.288、-0.456、-0.160、-0.423,P<0.05),与TC、HDL-C、LDL-C、N-MID、PINP及β-CTX正相关(r=0.170、0.366、0.135、0.625、0.583、0.601,P<0.05)。线性回归分析结果显示,调整年龄后,T2DM患者血清sFRP5可负向影响HbA1c、FPG、2hPG、FINS、HOMA-IR、TG、BMD(β分别为-0.115、-0.170、-0.402、-0.224、-0.139、-0.029、-0.039,P<0.05),正向影响TC、HDL-C、LDL-C、N-MID、PINP、β-CTX(β分别为0.038、0.041、0.019、0.408、1.065、0.021,P<0.05)。结论:血清sFRP5水平与T2DM患者糖脂代谢、骨代谢关系密切,可能参与T2DM患者骨质疏松的发病过程。
Aim:To explore the correlation between serum secretory frizzled protein 5(SFRP5)and glucose and lipid metabolism,bone metabolism and bone mineral density in type 2 diabetes mellitus(T2DM)patients.Methods:A total of 150 T2DM patients were selected as the T2DM group,and 108 patients with OGTT test as normal glucose tolerance(NGT)were the NGT group.Serum SFRP5 level,glucose and lipid metabolism indexes[fasting blood glucose(FPG),hemoglobin A1c(HbA1c),fasting serum insulin(FINS),insulin resistance index(HOMA-IR),total cholesterol(TC),triglyceride(TG),low density lipoprotein cholesterol(LDL-C),high lipoprotein cholesterol(HDL-C)],bone metabolism indexes[N-terminal osteocalcin(N-MID),procollagen typeⅠN-terminal propeptide(PINP),C-terminal peptideβdegradation product(β-CTX)of typeⅠcollagen]and bone mineral density(BMD)were analyzed.Results:HbA1c,FPG,2hPG,FINS,HOMA-IR,TG,LDL-C,BMD in T2DM group were higher than those in NGT group,the levels of HDL-C,SFRP5,N-MID,PINP andβ-CTX in T2DM group were lower than those in NGT group(P<0.05).Partial correlation analysis showed,after removing the age effects,serum sFRP5 negatively correlated with HbA1c,FPG,2hPG,FINS,HOMA-IR,TG,and BMD in T2DM group(r=-0.439,-0.436,-0.448,-0.288,-0.456,-0.160,-0.423,P<0.05),and positively correlated with TC,HDL-C,LDL-C,N-MID,PINP andβ-CTX(r=0.170,0.366,0.135,0.625,0.583,0.601,P<0.05).Linear regression analysis revealed that in T2DM group serum sFRP5 could negatively affect HbA1c,FPG,2hPG,FINS,HOMA-IR,TG and BMD(βwas-0.115,-0.170,-0.402,-0.224,-0.139,-0.029,-0.039,P<0.05),and positively affect TC,HDL-C,LDL-C,N-MID,PINP,β-CTX(βwas 0.038,0.041,0.019,0.408,1.065,0.021,P<0.05).Conclusion:Serum SFRP5 level is closely related to glucose and lipid metabolism,bone metabolism in patients with T2DM,and may be involved in the pathogenesis of osteoporosis in patients with T2DM.
